摘 要:目的系统评价安氏Ⅱ2错畸形与个别正常的下颌骨形态特征的差异。方法计算机检索PubMed、Embase、CBM数据库并同时辅以手工检索搜集国内外学者公开发表的有关安氏Ⅱ2错畸形的颌骨形态特征的文章。检索时间从建库至2013年11月。由两位评价员独立评价进行资料提取和交叉核对后,采用Rev Man 5.1进行Meta分析。结果共纳入6篇文献,均为安氏Ⅱ2错畸形与个别正常对照的研究,其中安氏Ⅱ2错畸形的患者277例,个别正常共191例。Meta分析结果显示试验组的ANS-Me距离[MD=-2.42,95%CI(-3.43,-1.41),P<0.01]、Go角[MD=-3.74,95%CI(-5.04,-2.45),P<0.01]、Go-Me距离[MD=-2.32,95%CI(-3.68,-0.95),P=0.009]、N-Me距离[MD=-3.12,95%CI(-5.49,-0.74),P=0.010]及SNB角[MD=-2.68,95%CI(-4.67,-0.70),P=0.008]较对照组小;而试验组Pog-NB距离[MD=1.27,95%CI(0.89,1.65),P<0.01]较对照组大;以上的差异均具有统计学意义。两组Y轴角[MD=0.17,95%CI(-2.40,2.74),P=0.900]的差异无统计学意义。结论现有研究显示在安氏Ⅱ2错畸形中,其面部高度发育不足;下颌体的长度发育不足;下颌齿槽座点发育不足;下颌角较平缓;下颌骨颏部更突。Objective To systematically evaluate the differences in mandibular morphology characteristics between class Ⅱ^2 malocclusion with normal occlusion. Methods The computer retrieval from databases of PubMed,Embase and CBM and supple- mented manual retrieval were performed for collecting the relevant articles published at home and abroad. The retrieval time was from the database establishment to November 2013. The quality evaluation and data extraction were independently conducted by two authors. Then the meta analysis was performed by using the Rev Man 5.1 software. Results A total of 6 articles were included, which were the control trials on class Ⅱ^2 malocclusion and individual normal occliasion. 277 patients were the class Ⅱ^2 malocclusion and 191 patients were individual normal occlusion. The Meta analysis results showed that the ANS-Me distance[MD=- 2.42,95 CI(-3. 43,-1. 41) ,P〈0.01] ,Go angle[MD= -3.74,95 %CI(-5.04,-2.45) ,P〈0.01] ,Go-Me distance[MD= -2.32,95% CI(-3.68,-0. 95),P=0. 009], N-Me distanee[MD= - 3. 12,95%CI(-5.49,-0.74),P=0. 010] and SNB angle[MD= -2.68,95 % CI(- 4.67,-0.70), P= 0. 008] in the experiment group were smaller than those in the control group, while the Pog- NB distance in the experiment group was greater than that in the control groups[MD = 1.27,95% CI (0.89,1.65), P〈 0.01], the differences between the two groups were statistically significant. There was no statistically significant difference in the Y axis angle between the two groups[MD= 0.17,95 %CI(--2.40,2.74), P = 0. 900]. Conclusion The existing trials indicate that in the class Ⅱ^2 malocclusion, the face is highly deficient ;the mandibular body length is deficient ; the mandibular base point is deficient ;the mandibular angle is gentler ; mandibular symphysis is more prominent.
