高血压并发急性脑卒中患者心电图变化临床研究

摘　　要：目的探讨高血压并发急性脑卒中患者心电图的变化及临床意义。方法选取2008-01—2010-03于我院就诊的120例高血压并发急性脑卒中患者为观察组,未并发急性脑卒中患者106例为对照组,比较2组患者异常心电图检出率和表现形式,分析异常心电图检出率与脑卒中患者临床预后的关系。结果观察组患者心电图异常的发生率为85%,高于对照组的54.72%;观察组ST-T改变比率为48.33%,高于对照组的18.87%;Q-T延长的比率为27.50%,高于对照组的13.21%;心律失常为38.33%,高于对照组的21.70%;房室肥大、左心室高电压发生率为20.83%,高于对照组的11.32%;高血压并发急性脑卒中患者病情恶化、死亡患者异常心电图的检出率为97.22%,病情稳定好转患者异常心电图的检出率为77.50%,差异均有统计学意义(P<0.05)。结论高血压并发急性脑卒中会导致异常心电图,且会使心电图异常的类型复杂多样;临床上通过心电图的变化可以推断急性脑卒中的病变程度和预后情况。Objective To investigate ECG changes and clinical significance for patients with hypertension complicated with acute stroke.Methods 120 cases of acute stroke patients with hypertension was observed from January 2008 to March 2010 in our hospital ,106 cases without acute stroke are regarded as control group. The detection rate of abnormal and manifestations in two groups was analyzed ,as well as the relationship between the detection rate of abnormal ECG and clinical prognosis of stroke patients.Results The incidence 85% of ECG abnormalities in observation was higher than 54 .72% in control group. The rates of ST‐T changes , QT prolonged , arrhythmia , atrioventricular hypertrophy respective were 48.33% ,27.50% ,38.33% ,20.83% ,＆nbsp;which were higher than that 18.87% ,13.21% ,21.70% ,11.32% in control group. The rate of condition deteriorated and death with abnormal ECG was 97.22% ,the detection rate of abnormal ECG for patients with stable condition was 77.50%. The differences of the all the above data were statistically significant.Conclusion Hypertension with acute stroke results in abnormal ECG and make electro‐cardiogram complicated;ECG changes can be useful for the severity and prognosis of acute stroke.
