中药汤剂保留灌肠联合腹水超滤回输治疗肝硬化腹水的疗效观察

摘　　要：目的：观察自拟中药汤剂保留灌肠联合腹水超滤回输治疗肝硬化难治性腹水的疗效及安全性。方法选择2013年～2014年我院收治肝硬化难治性腹水患者133例，在内科治疗基础上增加中药保留灌肠、腹水超滤回输，分别于治疗前后观察患者临床症状、体重、腹围、24 h尿量、血清尿素和肌酐、血清电解质、血清蛋白、尿钠等指标的变化。结果治疗后临床症状明显改善，体重减轻，腹围缩小，尿量增加，尿钠和血清蛋白增高差异有统计学意义（P＜0.05）。血清谷丙转氨酶、谷酰胺转酞酶、谷草转氨酶、尿素和肌酐下降，与治疗前比较差异有统计学意义（P＜0.05），血清电解质差异无统计学意义（P＞0.05）。133例患者中，显效78例，有效47例，总有效率87.15%。结论自拟中药汤剂保留灌肠联合腹水超滤回输腹腔治疗肝硬化难治性腹水明显优于单纯用药及腹水超滤回输。Objective To investigate the effect and safety of traditional Chinese medicine and Western medicine retention enema combined with ascites reinfusion treatment of liver cirrhosis with refractory ascites. Methods From 2013 to 2014 in our hospital 133 cases of liver cirrhosis with refractory ascites in patients treated in the Department of internal medicine, based on the increase of retention enema with traditional Chinese medicine, to observe the changes of ascites ultrafiltration reinfusion, patients with clinical symptoms, body weight, waist circumference, 24h urine volume, serum urea and creatinine, serum electrolyte, serum albumin, urinary sodium index on before and after the treatment respectively. Results The clinical symptoms improved significantly after treatment, body weight, abdomen circumference, urine volume increased, urinary sodium and serum protein （P〈0.05）. Serum alanine aminotransferase, glutamine transpeptidase, aspartate aminotransferase, urea and creatinine decreased, compared with before treatment, signiifcant difference （P〈0.05）, no signiifcant difference in serum electrolyte （P〉0.05）. In 133 cases, 78 cases markedly effective, effective in 47 cases, the total efifciency of 87.15%. Conclusion The self-made traditional Chinese medicine retention enema decoction combined with ascites reinfusion treatment of liver cirrhosis with refractory ascites is better than simple drug and ascites ultrafiltration reinfusion.
