摘　　要：目的研究葡萄糖调节蛋白78(GRP78)在胃癌、慢性萎缩性胃炎及浅表性胃炎组织中的表达及临床意义。方法 RT-PCR法检测胃癌、慢性萎缩性胃炎和浅表性胃炎各25例新鲜组织中GRP78基因表达;免疫组化法检测胃癌、慢性萎缩性胃炎和浅表性胃炎各60例组织中GRP78蛋白表达。分析GRP78蛋白表达与临床病理参数间的相关性。结果胃癌组织中GRP78mRNA的表达水平高于慢性萎缩性胃炎和浅表性胃炎(1.26±0.18vs.0.89±0.25和0.29±0.09)(P<0.01)。胃癌组织中GRP78蛋白的阳性表达率亦高于慢性萎缩性胃炎和浅表性胃炎(78.3%vs.46.6%和6.7%)(P<0.01)。胃癌组织中GRP78蛋白表达与胃癌分化程度、分期、淋巴结转移等明显相关(P<0.05或P<0.01)。结论 GRP78在胃癌中呈高表达;检测GRP78的表达可能有助于预防及早期诊断胃癌。Objective To investigate the expression and clinical significance of glucose-regulated protein 78（GRP78） in the patients with gastric carcinoma ,chronic atrophic gastritis and superficial gastritis .Methods The expressions of GRP78 mRNA in fresh tissues of gastric carcinoma （n= 25） ,chronic atrophic gastritis （n= 25） and superficial gastritis （n= 25） were detected by RT-PCR .The expressions of GRP78 protein in gastric carcinoma tissues （n= 60） ,chronic atrophic gastritis tissues（n= 60） and superficial gastritis tissues （n= 60） were detected by immunohisto-chemistry assay .The relationship between GRP78 expression and age ,gender ,differentiation , invasion ,disease stage and lymphoid node metastasis was analyzed .Results The expression level of GRP78 mRNA in gastric carcinoma tissues was significantly higher than that in the tissues of chronic atrophic gastritis and superficial gastritis（1.26 ± 0.18 vs .0.89 ± 0.25 and 0.29 ± 0.09）（P〈0 .01） .So did the positive ratio of GRP78 protein expression（78.3% vs .46.6% and 6.7% ）（P〈0 .01） .GRP78 expression was positively correlated with tumor differentiation ,disease stage and lymphoid node metastasis（P〈0 .05 or P〈0 .01） .Conclusion GRP78 is highly expressed in the gastric carcinoma . Detection of GRP78 in tumor may be helpful for the prevention and early diagnosis of gastric carcinoma .
