摘  要:目的探讨多层螺旋CT及重建技术对气管主支气管肿瘤的诊断价值。方法 20例气管主支气管肿瘤患者,所有患者均经手术病理证实为原发性气管主支气管肿瘤。对患者的多层螺旋CT检查结果进行回顾性分析,在AW工作站,应用多平面重建(MPVR)、肺透明化重建(VR)和仿真内窥镜(CTVE)软件对图像重建数据行后处理重建,分别得到MPVR、VR和CTVE图像,将结果与手术病理结果进行对照分析。结果良性肿瘤2例,恶性肿瘤18例,11例分布在气管,4例分布在左主支气管,5例分布在右主支气管;管腔内窄基底结节6例,管腔内宽基底结节14例;生长不规则11例,成环状生长3例,偏心生长6例;6例向管腔内突出未向外浸润,4例轻度向外浸润,10例向外浸润并与相邻结构粘连;平滑肌肉瘤密度均匀,中度强化,脂肪瘤为低密度影,不强化,恶性肿瘤密度不均匀,强化不均匀。结论多层螺旋CT及重建技术能够对气管主支气管肿瘤做出准确的诊断并帮助制定手术方案,具有重要的临床意义。Objective To investigate the diagnostic value of multi-slice spiral CT and reconstruction technique for trachea and principal bronchus tumor. Methods There were 20 patients with trachea and principal bronchus tumor, and all patients were diagnosed as primary trachea and principal bronchus tumor by surgery and pathology. Multi-slice spiral CT results were retrospectively analyzed. In AW station, multi planar volume reconstruction (MPVR), volume rendering air structure (VR), and computed tomographic virtual endoscopy (CTVE) were applied for image reconstruction. The results of MPVR, VR, and CTVE images were used for comparative analysis with surgery and pathology. Results There were 2 cases with benign tumor and 18 cases with malignant tumor;11 cases in trachea, 4 cases in left principal bronchus, and 5 cases in right principal bronchus; 6 cases with luminal narrow base node, 14 cases with luminal wide base node; 11 cases with irregular growth, 3 cases with annular growth, 6 cases with eccentric growth; 6 cases with luminal extrude, 4 cases with mild outward infiltrate, 10 cases with outward infiltrate and adhesion to adjacent structures. Leiomyosarcoma had uniform density and moderate intensify. Lipomyoma had low density shadow without intensify. Malignant tumor had uneven density and uneven intensify. Conclusion Multi-slice spiral CT and reconstruction technique can provide accurate diagnosis and establishment of operation plan for trachea and principal bronchus tumor, and this method contains important clinical significance.
