下颌骨牙源性角化囊性瘤的影像诊断

摘　　要：目的：探讨下颌骨牙源性角化囊性瘤(KCOT)的 X 线和 CT 表现特点,提高对该病的诊断水平。方法回顾性分析经手术和病理证实的25例下颌骨 KCOT 的 X 线和 CT 表现。结果单发 KCOT 23例,多发 KCOT 2例,主要表现为下颌骨内单房或多房囊状骨质破坏区,边缘清楚光滑伴有硬化,7例局部骨皮质缺损,18例较大病变沿下颌骨长轴生长,21例邻近牙齿受压移位,2例牙根呈斜面状吸收。结论下颌骨 KCOT 的影像表现多具有特征性,有助于诊断及鉴别诊断。Objective To discuss the X-ray and CT manifestations of keratocystic odontogenic tumour(KCOT)in mandible,to im-prove the diagnostic accuracy of the disease.Methods The X-ray and CT features of KCOT(n=25)in mandible,which were proved by surgery and pathology,were retrospectively analyzed.Results The KCOT,including solitary tumor(n=23)and multiple tumor (n=2),mainly displayed unilocular or multilocular in shape with distinct sclerosing margin.Disruption of the adjacent cortex was de-tected in 7 cases,growth along the long axis of mandible in 18 cases,compression and displacement of the adjacent teeth in 21 cases, resorption of bevel of roots in 2 cases.Conclusion Most lesions of KCOT in mandible have characteristic manifestation in X-ray and CT findings,which is helpful for diagnosis and differential diagnosis.
