含莫西沙星四联方案对幽门螺杆菌根除初次治疗的疗效研究

摘　　要：[目的]观察含莫西沙星四联疗法在幽门螺杆菌(Hp)感染根除初次治疗中的临床疗效和不良反应。[方法]将120例Hp感染患者随机分为A组和B组,每组均为60例。A组给予口服莫西沙星(400mg/次,1次/d)+阿莫西林(1g/次,2次/d)+埃索美啦唑(20mg/次,2次/d)+胶体果胶铋(200mg/次,2次/d);B组给予克拉霉素(500mg/次,2次/d)+阿莫西林(1g/次,2次/d)+埃索美啦唑(20 mg/次,2次/d)+胶体果胶铋(200 mg/次,2次/d)。2组均治疗10d,2组停服药物4周后行13 C-BUT试验检测Hp感染是否根除。[结果]按意向治疗分析(ITT)A组、B组的Hp根除率分别为85.00%、80.00%,按符合方案分析(PP)A组、B组的Hp根除率分别为91.07%、87.27%,2组比较均差异无统计学意义(P=0.471、P=0.519)。A组、B组的不良反应率分别为11.7%、8.3%,2组比较差异无统计学意义(P=0.543)。[结论]含莫西沙星的四联疗法是一种安全、有效的根除Hp感染的方案,可以作为初次治疗方案之一。[Objective]To evaluate the efficacy and adverse‐effect of moxifloxacin‐based quadrpule thera‐py as initial treatment for Helicobacter pylori（H p）infection.[Methods]The total of 120 patients were randomly divided into 2 groups ：groups A （n=60）received oral moxifloxacin 400 mg qd＋amoxicillin 1.0 g bid＋esomeprazole 20 mg bid＋pectin bismuth 200 mg bid ;group B（n=60）received oral clarithromycin 0.5 g bid＋amoxicillin 1.0 g bid＋ esomeprazole 20 mg bid＋ pectin bismuth 200 mg bid.The patient in both groups were treated for 10 days.The eradication rates of H p were evaluated by 13 C‐urea breath test 4 weeks after the treatment.[Results]The rates of H p eradication in groups A and B were 85.00% and 80.00% by intention‐to‐treat analysis ,which there was no significant difference（P=0.471）.The rates of H p eradication in groups A and B were 91.07% and 87.27% by perprotocol analysis ,which there was no significant difference（P= 0.519）.The rate of adverse‐effect of the two groups were 11.7% and 8.3% , which there was no significant difference（P=0.543）.[Conclusions]Moxifloxacin‐based quadrpule was an effective and safe choice as initial treatment for H p infection.
