细胞角蛋白在毛囊皮脂腺囊性错构瘤中的表达及临床意义

摘　　要：目的观察细胞角蛋白(cytokeratin,CK)CK34βE12,CK5/6,CK7,CK8/18,CK15,CK17,CK19和CK20在毛囊皮脂腺囊性错构瘤(folliculosebaceous cystic hamartoma,FSCH)中的表达,并探讨其临床意义。方法以石蜡包埋的5例FSCH皮损组织标本作为研究对象,使用全自动免疫组化染色仪进行检测,对5例FSCH进行上述CK标记,观察不同细胞角蛋白的表达和分布模式。结果 CK34βE12,CK5/6,CK7和CK17在FSCH皮损中呈阳性表达,而CK8/18,CK15,CK19和CK20呈阴性表达。结论 CK7和CK17在FSCH中呈阳性表达,而CK15和CK19表达缺失,提示其向毛囊漏斗部和皮脂腺分化;有选择进行CK的联合检测,可有助于FSCH的诊断及推测其分化方向。Objective To investigate the immunohistochemical expression of eight cytokeratins in FSCH and to evaluate its clinical significance. Methods Eight cytokeratins including CK3413E12, CK5/6, CK7, CK8/18, CK15, CK17, CK19 and CK20 were detected in 5 FSCH cases by immunohistochemistry. Their expressions were meas- ured by ventana automatic immunohistochemistry stainer. The expression and distributed patterns of cytokeratins were observed. Results CKJ413E12, CK5/6, CK7 and CK17 were positive expression, and CK8/18, CK15, CK19 and CK20 were negative in all FSCH cases. Conclusion The positive staining for CK7 and CK17,and the lack of K15 or K19 in FSCH indicates its differentiation toward the infundibulum and sebaceous gland. Combined detection with CKs, contributing to diagnosis of FSCH and proposing its differentiation.
