轻型颅脑损伤后血清S100β浓度与头颅CT及预后的相关性研究

摘　　要：目的通过检测格拉斯哥昏迷量表（GCS）评分在13~15分的患者血清S100β蛋白含量,比较头颅CT阴性的轻型颅脑损伤（m TΒI）患者和头颅CT阳性的颅脑损伤（TΒI）患者血清S100β浓度与预后的关系,为临床m TBI患者的诊治和判断预后提供依据。方法用ELISA法检测64例TBI患者血清S-100β含量,并与20例体检健康者比较。对比29例m TBI患者和35例TΒI患者血清S-100β含量。评估患者6个月后的延伸格拉斯哥预后量表（GOS-E）评分,对其与血清S-100β含量的关系进行相关分析。结果 m TBI患者血清S-100β含量高于正常对照组（P〈0.05）,TBI患者血清S-100β含量又高于m TBI组（P〈0.05）;血清S-100β含量对鉴别TΒI后头颅CT阳性和阴性患者有一定准确性。血清S-100β含量与m TBI患者的预后无相关性（P〉0.05）,但是与TBI患者的预后呈负相关（P〈0.05）。结论血清S-100β可为临床诊断m TBI提供帮助,对预测TBI患者头颅损伤可能有一定准确性。血清S-100β不能判断m TBI患者的预后;但对于TBI患者,血清S100β越高则预后可能越差。Objective To compare the concentrations of serum S 100βin patients of mild traumatic brain injury（mTBI） with the cranial CT negative cranial CT positive and explore the relationship between S100βand prognosis by examining of the content of serum S 100βprotein in patients with GCS score 13-15, in order to provide basis for clinical diagnosis , treatment and prognosis of patients with mTBI . Methods Serum S100βcontent was detected by ELISA and comparative study in 64 cases of TBI patients and 20 healthy subjects was perpormed.the serum concentrations of S100βwas compared between the 29 cases of mTBI patients and the 35 cases of TΒI patients.the relationship was assessed between the GOS-E score of patient after 6 months and the S100β.Results The Serum content of S100βin the mTBI group was higher than that in the control group （P〈0.05）, and serum content of S100βin the TBI group was higher than that in mTBI group （P〈0.05）.It has a certain accuracy to discriminate between the patient with mTBI and those with TBI by the serum S 100βcontent .There was not relationship between the GOS-E score of patient with mTBI after 6 months and the S100β（ P〉0.05）, but in the patient with TBI , the serum S100βcontents correlated with the prognosis of the patient （P〈0.05）.Conclusion Serum S100βcontent may be used as diagnostic biomarkers of mTΒI, It can found the patients with brain injury ,but it can not predict the prognosis of patients with mTBI, maybe in the patient with TBI , the higher serum S100βcontent, the worse the prognosis .
