摘 要:目的评价正常小型猪肾脏纳米刀消融后的影像及病理学变化特点。方法实验用巴马小型猪5只,采用CT导向经皮穿刺术对肾脏预定靶区进行纳米刀消融,分别于消融术后2h、2周、4周、8周、16周对每只实验猪行腹部CT增强扫描,并于每次扫描后随机处死1只用以病理分析。结果对5只小型猪均消融成功,共形成9个消融区域。CT增强扫描图像显示术后2h肾脏消融区域均可见局限性强化减低,病理图显示肾小球结构破坏,肾小管上皮细胞凋亡,血管及肾小管结构大致完整;术后2、4、8、16周消融区域与未消融区域无明显差别,均未见集合系统损伤,病理图显示纤维组织增生,少量肾小管上皮细胞再生。结论纳米刀消融猪肾脏后,急性期对肾脏结构有一定损伤,随时间延长,肾脏损伤逐渐修复。Objective To evaluate the changes of miniature pig kidney imaging and pathology after ablation of NanoKnife. Methods Five Bama miniature pigs were used in the experiment. NanoKnife ablation probe was placed into predetermined target of renal parenchyma by CT guided percutaneous puncture technology for irreversible electroporation ablation. Ab- dominal CT enhanced scan were carried out respectively 2 h, 2 weeks, 4 weeks, 8 weeks, 16 weeks after ablation, and 1 experimental pig was killed after each scan for pathology analysis. Results Five miniature pigs were punctured successful- ly. Total nine ablation areas were formed. CT enhancement scan showed local reduced enhancement areas 2 h after abla- tion. Pathology showed glomerular structure was damaged 2 h after ablation. Renal tubular epithelial cell apoptosis were observed. Vascular and tubular structure were substantially integrated. CT enhancement scan showed the ablation areas were no difference with non-ablated areas 2, 4, 8, 16 weeks after ablation, no damage of collection systems were observed. Pathology showed fibrosis and a small amount of regeneration of renal tubular epithelial cells were visible on later. Conclu- sion The structure of renal have some damage after ablation of pig kidney in acute phase, the damage of kidney are re- paired gradually later.
