摘  要:目的评价纳米刀消融技术应用于活体猪肺组织的有效性与安全性。方法取健康巴马小型猪5只(实验动物A、B、C、D、E)用于实验,于CT引导下经皮穿刺纳米刀消融猪肺组织,分别于消融后1天、2周、1个月、2个月、4个月随机抽取1只行肺部CT扫描后处死实验动物,整体切除右肺并制作病理切片进行组织学观察。结果对所有实验动物均顺利完成消融。实验动物E、F出现少许气胸,余未见并发症。CT图像示消融区与周围组织分界较清晰,内部支气管、血管管腔形态完整。消融后1天消融区呈磨玻璃密度;消融后2周消融区密度不均匀增高,范围减小;消融后1个月及2个月高密度范围增大且密度较前减低;消融后4个月消融区范围减小,密度较均匀。各时间点病理切片均可见不同程度肺水肿及炎性浸润。术后1天、2周炎细胞浸润及肺水肿明显,1个月、2个月及4个月可见大量纤维增生及明显再生修复。结论应用纳米刀技术对猪肺组织进行消融安全、有效。Objective To evaluate the efficacy and safety of NanoKnife ablation in treatment of lung tissue lesion in pig models. Methods A total of 5 healthy Bama mini-pigs (A, B, C, D and E) were studied. Ablation of the lung using NanoKnife was performed under the guidance of CT, And then the lung was scanned by CT 1 day, 2 weeks, 1 month, 2 months and 4 months after ablation. One pig was sacrificed randomly on each time point after CT scan. The right lung was resected and used as section for pathological examination. Results Ablation was performed successfully in each experimen- tal animal. Mild pneumothorax was found in animal E and F, respectively. No complication was found in other animals. The boundary between ablated and normal lung tissue was clear on CT images. The morphology and orientation of the in- ternal blood vessels and bronchi were unabridged. Ablation area showed as ground glass opacity 1 day after ablation. The density of ablation area increased unequally, as well as the range of ablation area decreased 2 weeks after ablation. The ab- lation area showed larder range and lower density 1 and 2 months after ablation. The decreased range and uniform density of ablation area were found 4 months after ablation. Inflammatory ceil infiltration and pulmonary edema were found in each time point, and were observed on 1 day and 2 weeks after ablation, Significant fibrosis and regeneration were detected on 1 month, 2 months and 4 months after ablation. Conclusion NanoKnife ablation is effective, safe and practicable in treat- ment of lung lesions in pig models.
