磁共振壳核面积测量对帕金森病和纹状体黑质变性的鉴别诊断意义

摘　　要：目的通过MRI壳核面积测量为纹状体黑质变性(SND)和帕金森病(PD)两种临床容易混淆的疾病提供了一个可靠的鉴别方法。方法回顾性分析经临床诊断为SND 9例,PD17例和正常人17例常规磁共振成像资料,通过MRI对壳核面积进行测量。结果 SND患者的壳核面积较PD患者及正常组均显著降低(P﹤0.05),而PD患者和正常组壳核面积差别无显著意义。SND主要表现为在T2加权像可见壳核边缘高信号和壳核萎缩,部分患者可见桥脑十字征。轴位T1加权像可见小脑中脚萎缩等MRI表现,矢状位T1加权像可见小脑、桥脑和延髓萎缩等MRI表现。结论MRI壳核面积测量有助于SND和PD两种疾病的鉴别诊断。Objective We have provided a reliable differential diagnosis for the striatonigral degeneration （SND） and Parkinson disease （PD） that are easy to confuse disease in clinical diagnosis by MRI measurement .Methods The clinical diagnosis for the SND of 9 cases ,PD of 17 cases and 17 normal cases of conventional magnetic resonance imaging data are retrospectively analyzed .Putamen area have been measured by the method of measuring the M RI .Results Putamen area is statistically significant （ P﹤0 .05） to be found compared with PD group and control group .But the PD group and con‐trol group have no statistically significant difference in the putamen area .The main MRI findings of SND are hyperintense rim of putamen and putaminal atrophy .The cross sign of the T2 WI is found in the few SND patiens .The cerebellar atro‐phy ,atrophy of middle cerebellar peduncles ,medulla oblongata atrophy and pontine atrophy are found in the few SND pa‐tiens .Conclusion M RI measurements of the putamen area helps in the differential diagnosis of SND and PD .
