摘 要:目的探讨过氧化氢酶(CAT)和超氧化物歧化酶(SOD)对冠心病患者估算肾小球滤过率(eGFR)的影响。方法选取行冠状动脉造影术证实冠心病的患者280例,测定eGFR、血糖、肾功能、血脂、CAT和SOD等。根据造影结果,采用Gensini评分对每支血管病变进行定量评分并进行分析。结果 CAT、SOD活性与简化肾脏病膳食改良(MDRD)-eGFR呈显著正相关(r=0.634,P=0.006;r=0.791,P=0.002),校正年龄、体质量指数、血压、血脂、血糖等因素后,GAT和SOD活性与MDRD-eGFR仍呈正相关(P<0.05)。随着eGFR的下降,CAT及SOD水平呈逐渐下降趋势(P<0.01)。多元线性回归分析显示,CAT对eGFR的影响更大。结论冠心病患者血浆CAT、SOD活性与肾功能具有正相关性,且CAT对eGFR的影响更大。Objective To study the effect of catalase(CAT)and superoxide dismutase(SOD)on eGFR in coronary heart disease(CHD)patients.Methods Two hundred and eighty angiographyconfirmed CHD patients were included in this study.Their eGFR and renal function were tested,and their serum levels of blood glucose,blood lipid,CAT and SOD were measured.The Gensini scores of lesions in each blood vessel were quantitatively analyzed.Results The activity of CAT and SOD was positively related with the MDRD-eGFR(r=0.634,P=0.006;r=0.791,P=0.002)and even after adjustment for age,BMI,blood pressure,blood lipid,blood glucose(P0.05).The serum CAT and SOD levels decreased with the decreasing eGFR(P0.01).Multivariate regression analysis showed that CAT exerted a greater impact on eGFR than SOD.Conclusion The activity of plasma CAT and SOD is positively related with eGFR in CHD patients and CAT exerts a greater impact on eGFR than SOD.
