两种不同材料套筒冠义齿修复轻中度牙列缺损的疗效分析

摘　　要：目的:应用两种不同修复材料套筒冠义齿修复轻中度牙周病伴牙列缺损,比较临床疗效。方法:20例轻中度牙周病伴牙列缺损患者,进行完善根管治疗及牙周基础治疗后设计制作27副圆锥型套筒冠义齿,其中13副使用金沉积内外冠固位,14副使用氧化锆内冠及金沉积外冠固位。基牙共120颗。随访3年比较修复前后基牙牙周探诊深度(PD),基牙松动度、骨高度及骨密度等。结果:20例患者共27副圆锥型套冠义齿使用良好,牙齿松动度及牙周探诊深度变小(P<0.05),患者满意度明显高于卡环式可摘局部义齿。义齿使用2年后3例金沉积套筒冠义齿固位力下降,而氧化锆内冠及金沉积外冠套筒冠义齿固位良好,无下降趋势。结论:不同材料的套筒冠义齿对轻中度牙周病伴牙列缺损病例具有良好的临床疗效。利用氧化锆内冠和金沉积外冠固位的套筒冠义齿持久固位力优于金沉积内外固位义齿。Objective To compare two kinds of sleeve different repair materialsdenture clinical defect cases were mild to moderate periodontal disease withteeth. Methods 20 patients with mild to moderate periodontopathy with dentitiondefect patients,perfect design and making 27 pairs of conical telescopic removable denture root canal treatment and periodontal treatment,of which 13pairs of gold deposition inside and outside the crown retention,14 vice usingzirconia within the crown and gold deposition crown retention. A total of 120abutment teeth.3 years of follow-up comparison before and after the restoration of abutment periodontal probing depth （PD）,loose abutment, bone height and bone density. Results 20 cases of patients with a total of 27 pairs of conicalsleeve denture use is good,loosening of the teeth and periodontal probing depth becomes small （P〈0.05）,the degree of satisfaction of patients was significantly higher than that of clasp removable partial denture. Drop denture use after 2 years in 3 cases of cobalt chromium alloy telescopic denture and 1 cases of gold deposition telescopic denture retention,and 4 cases of zirconium coping and deposition of gold crown telescopic denture retention is good, no downward trend. Conclusion The different materials of telescopic crown denture defectcases has good curative effect on treating mild to moderate periodontal disease with teeth. The use of zirconia crowns and deposition of gold crown retentivetelescopic denture lasting retention force of cobalt chromium alloy telescopic denture.
