超声造影和MRI诊断小肾癌的对比

摘　　要：目的探讨CEUS和MRI对小肾癌的诊断价值。方法收集可疑肾实性小结节患者32例。对所有患者行CEUS和MR检查,观察小肾癌的CEUS图像特点,评价CEUS和MRI对小肾癌的诊断效能。结果 CEUS示假包膜环状增强19例(19/21,90.47%),动脉期同/快进14例(14/21,66.67%),均匀增强14例(14/21,66.67%)。32例患者中,小肾癌21例,CEUS测量的结节大小[(1.95±0.62cm)]与MRI测量结果[(1.87±0.69)cm]差异无统计学意义(t=1.6132,P>0.05)。CEUS诊断小肾癌的敏感度、特异度、阳性预测值、阴性预测值和准确率分别为85.71%(18/21)、81.82%(9/11)、90.00%(18/20)、75.00%(9/12)和84.36%(27/32),MRI为90.48%(19/21)、90.91%(10/11)、95.00%(19/20)、83.33%(10/12)和90.63%(29/32)。结论 CEUS对无症状的小肾癌具有重要的诊断价值,对于MR检查受限的患者,CEUS是可选择替代的较为准确的影像学诊断方法。Objective To explore the value of CEUS and MRI in diagnosing of small renal carcinoma. Methods Thirty two patients were suspected with small renal solid lesions. All patient was underwent both CEUS and MRI. The image characteristics of CEUS were observed, and both CEUS and MRI diagnostic value were evaluated. Results In CEUS ima- ges, 19 cases （19/21, 90.47%） showed rim-like enhancement of pseudocapsule, 14 cases （14/21, 66.67%） showed same or quick wash-in and 14 cases （14/21,66.67%） showed homogeneous enhancement. Twenty one patients were finally diag- nosed with small renal carcinoma. There was no statistically difference of lesion size measured by CEUS （]1.95±0. 62]cm） and MRI [1.87±0. 697cm; t= 1. 6132, P〉0.05）. The diagnostic sensitivity, specificity, positive predictive value, nega- tive predictive value and the coincidence rate of CEUS for small renal cancer were 85.71% （18/21）, 81.82% （9/11）, 90.00% （18/20）, 75.00% （9/12） and 84.36% （27/32）, of MRI were 90.48%（19/21）, 90.91% （10/11）, 95.00% （19/20）, 83.33% （10/12） and 90.63%（29/32）. Conclusion CEUS has significant value in diagnosing asymptomatic small renal cancer, and it may be an alternative method for those who can not take MRI.
