摘 要:目的探讨膀胱癌组织中CD4^+、CD8^+及CD20^+淋巴细胞浸润情况及其与膀胱癌病理分级和临床分期的关系。方法应用免疫组织化学二步法检测34例膀胱癌患者组织中CD4、CD8及CD20分子的表达,并分析其与临床病理特征的关系。结果 34例膀胱癌组织中,CD4、CD8和CD20染色阳性率分别为100.0%、85.3%和64.7%,CD4、CD8阳性率均明显高于CD20(P<0.05)。CD4分子在所有膀胱癌组织标本中均呈阳性表达;CD8分子在不同病理分级和临床分期组的表达差异均无统计学意义(P>0.05),但其阳性率随病理分级及临床分期的增加有下降的趋势;CD20分子在不同病理分级组中的表达差异无统计学意义(P>0.05),其阳性率随临床分期增加有升高的趋势,但差异无统计学意义(P>0.05)。结论膀胱癌组织以T淋巴细胞浸润为主,且CD8^+T细胞可能在其中发挥更为积极的作用,炎症细胞浸润可能是膀胱癌变过程中的早期事件。Objective To study the infiltration of CD4^+ , CD8^+ , CD20^+ lymphocytes in the tissue of bladder cancer and to in-vestigate their correlation with pathological grading and clinical stage. Methods CD4, CD8, CD20 expressions were analyzed by two - step immunohistochemistry staining technology in 34 cases of bladder cancer patients. Also, the relationship between expression of CD4, CD8, CD20 and clinical pathological characteristics. Results In the 34 cases of bladder cancer patients, the positive rates of CIM, CD8, CD20 were 100.0%, 85.3% and 64.7%, respectively. The positive rates of CD4, CD8 were both significantly higher than that of CD20 ( P 〈 0.05 ). CDd in all tissue specimens were positive. The expression difference of CD8 between different pathological grading and the different clinical stages was not statistically significant (P 〉 0.05), but the positive rate of CD8 presented a downward trend with pathological grading increasing or clinical stage increasing. The expression difference of CD20^+ lymphocyte was not statistical significant in different pathological grading patients( P 〉 0.05 ), and the posi- tive rate of CD20 presented upward trend with clinical stage increasing, however, the difference was not statistically significant (P 〉 0.05). Conclusion T lymphocyte may be the major infiltrating lymphocytes in the tissues of bladder cancer and the CD8^+ T cell may occupy an important position in the processing of bladder cancer, inflammatory cell infiltration in cancer tissue might be an early event in bladder cancer.
