胃印戒细胞癌组织中HER-2及CEA的表达与临床意义

摘　　要：目的探讨人类表皮生长因子受体基因（HER-2）及癌胚抗原（CEA）在胃印戒细胞癌组织中的表达及临床意义。方法选取80例胃印戒细胞癌患者作为实验组,40例萎缩性胃炎患者作为对照组。应用免疫组织化学法检测两组组织中HER-2及CEA的表达,并分析其表达与胃印戒细胞癌临床特征的关系。结果 HER-2及CEA在胃印戒细胞癌组织中的阳性表达率分别为43.8%和63.8%,明显高于萎缩性胃炎组,差异具有统计学意义（P〈0.01）。HER-2在2期～4期胃印戒细胞癌组织中的阳性表达率为70.5%,明显高于1期的阳性率（11.1%）,P〈0.05。有淋巴结转移组的HER-2阳性表达率（78.8%）明显高于无淋巴结转移组（19.1%）,P〈0.01。HER-2的表达与肿瘤大小、侵犯深度无关（P〉0.05）。CEA的表达与淋巴结转移相关（P〈0.05）,而与临床分期无关（P〉0.05）。结论 HER-2及CEA的表达可能与胃印戒细胞癌的发生、发展及淋巴结转移有关。Objective To study the expression and significance of HER -2 and CEA in gastric signet carcinoma. Methods 80 patients with gastric signet carcinoma and 40 patients of chronic atrophic gastritis were collected as the case group and control group respectively. Immunohistochemistry was used to detect the expression of HER- 2 and CEA, and the relation between the expression and clinic characteristics of gastric signet carcinoma was analyzed. Results The positive rates of HER - 2 and CEA expression in gastric signet carcinoma were 43.8% and 63.8% respectively, higher than that of atrophic gastritis, and the differences were statistically significant（ P 〈0. 01 ）. The positive expression rate of HER- 2 in the 2nd ～ 4th clinical stages was 70.5% , significantly higher than that in the first stage（ 11.1% ） , P 〈0.05. The positive expression rate of HER - 2 in gastric signet carcinoma with lymph node metastasis was 78. 8%, significantly higher than that without lymph node metastasis （ 19.1% ）, P 〈 0.01. There were no significant difference of HER- 2 expression in tumor size and infiltrate depth （P 〉 0.05 ）. There were correlations with lymph node metastasis（ P 〈 0.05 ）, but not correlated with clinical stages（ P 〉 0.05 ）. Conclu- sion The expression of HER -2 and CEA may facilitate the growth and lymph node metastasis of gastric signet carcinoma.
