乳腺癌中肉碱棕榈酰转移酶1的表达及其在乳腺癌发生发展中的作用

摘　　要：目的 观察乳腺癌中肉碱棕榈酰转移酶1（CPT1）的表达及其在乳腺癌发生发展中的作用.方法 采用免疫组织化学方法检测211例乳腺癌患者（中位年龄50.7岁）的癌组织及癌旁组织标本中CPT1蛋白的表达,探讨CPT1与临床病理特征之间的关系.结果 CPT1在乳腺癌组织中阳性表达140例（66.35％）,癌旁组织中阳性表达13例（6.16％）,两者差异有统计学意义（P＜0.01）.细胞组织学Ⅲ级、肿瘤最大径＞2 cm、淋巴结有转移、细胞核增殖抗原（Ki-67）阳性表达率≥14％、雌激素受体（ER）阳性表达及人类表皮生长因子受体-2（Her-2）阳性表达组CPT1阳性表达率显著高于细胞学分级Ⅰ～Ⅱ级（P＜0.01）、肿瘤最大径≤2cm（P＜0.01）、淋巴结无转移（P＜0.01）、Ki-67阳性表达率＜14％（P＜0.01）、ER阴性表达（P＜0.01）及Her-2阴性表达组（P＜0.01）,在发病年龄、月经状态及孕激素受体（PR）表达分组中差异无统计学意义（P＞0.05）.结论 乳腺癌组织中CPT1阳性表达率增高,CPT1能促进癌细胞增殖、侵袭及转移,与乳腺癌的发生、发展关系密切.Objective To investigate the expression of carnitine palmitoyl transferase 1 （CPT1）in breast cancer and study its role in the pathogenesis and development of breast cancer.Methods CPT1 expression in 211 cases of human breast cancer tissues and non-cancerous adjacent breast tissues was examined by immunohistochemistry.The relationship between the differential expression of CPT1 and the clinicopathological variables was analyzed as well.Results The expression rate of CPT1 was 66.35% in breast cancer tissues and 6.16% in non-cancerous adjacent breast tissues （P 〈 0.01）.The expression of CPT1 was significantly upregulated in patients with higher histological grade （P 〈 0.01）,larger tumor size （P 〈0.01）,axillary lymph nodes metastasis （P 〈0.01）,proliferation cell nuclear antigen （Ki-67） ≥14% （P 〈 0.01）,estrogen receptor （ER,P 〈 0.01） and human epidermalgrowth factor receptor-2（Her-2） positive status （P 〈 0.01）,but not relevant with age at diagnosis （P 〉 0.05）,menopausal status （P 〉 0.05）,and progesterone receptor （PR） status （P 〉 0.05）.Conclusion CPT1,up-regulated in the breast cancer tissue,could meet the high energy need of proliferation,invasion and metastasis,and is correlated with the pathogenesis and development of breast cancer.
