超声引导穿刺引脓联合脓腔抗菌药冲洗治疗颌面间隙感染

摘　　要：目的:探讨超声引导下穿刺引流脓液,同时脓腔内应用抗菌药冲洗治疗颌面间隙感染的疗效。方法:60例颌面间隙感染患者按入院时间先后分为两组,均采用超声引导下穿刺引流出颌面间隙脓液,同时脓腔内分别应用甲硝唑液和庆大霉素液治疗,观察脓腔及疼痛消失时间、血象、体温恢复正常时间,并比较两组患者有效率、治愈时间及治疗费用的差异。结果:庆大霉素液组患者白细胞恢复至正常范围时间、脓腔消失时间明显短于甲硝唑液组(P<0.05),治疗有效率、治愈时间及治疗费用指标也均优于甲硝唑液组(P<0.05)。治疗过程中两组均未见颌面间隙血肿、感染加重及窒息等并发症出现。结论:颌面间隙感染超声引导下穿刺引出脓液,配合脓腔内应用庆大霉素液,能够快速、有效治疗颌面间隙感染。Objective： To explore the effect of ultrasound-guided puncture treatment for maxillofacial space infection,in conjunction with antibiotic irrigation in pus cavity. Methods： Sixty cases involved by maxillofacial space infection were treated by ultrasound-guided puncture drainage,assisted by antibiotics irrigation. The patients were divided into two groups（ gentamicin and metronidazole） respectively by admission time,to observe the disappearing time of pus cavity and pain,the time of leukocyte and body temperature returned to normal level. The treatment effective rate,healing time and cost in different treatment method groups were compared. Results： Among the observed indicators of effective rate,healing time and cost,the gentamicin group got a better result compared with metronidazole group（ P〈0. 05）. None of the patient during the treatment process appears maxillofacial hematoma,infection exacerbation and asphyxia. Conclusion： Ultrasound-guided puncture drainage for maxillofacial space infection,assisted by antibiotics irrigation is a fast,safe,effective and economical treatment method.
