摘 要:目的应用Spectralis频域OCT(SD—OCT)测量分析视网膜中央静脉阻塞(CRVO)黄斑囊样水肿(CME)时黄斑区视网膜厚度及容积变化。方法临床病例系列研究。对2009年6月至2014年2月在河北医大二院眼科就诊的50例经眼底荧光血管造影诊断为CRVO—CME未经治疗患者50只眼行SD-OCT检查,以直径1、3、6mmETDRS对黄斑进行分区,分析黄斑区视网膜厚度、容积情况,并将之与对侧健眼或性别、年龄匹配眼对比、分析。结果CRVO—CME黄斑视网膜各ETDRS分区较对侧健眼或匹配眼均增厚,中央区最厚,呈峰位于中央区的山峰样,分区容积变化与厚度相一致;CRVO-CME黄斑中央区中心小凹视网膜厚度(FT)、中央区平均视网膜厚度(FMcme)、中央区最厚值(Fmax)及中央区视网膜容积(FV)与最佳矫正视力(BCVA)相关。结论CRVO—CME黄斑视网膜水肿增厚呈山峰样,较正常视网膜显著增厚,SD—OCT可以实现对黄斑区视网膜厚度容积的精确测量,对CRVO—CME的诊断及治疗提供有力依据。Objective To investigate the changes of macular thickness and cubic volume of cys- toid macular edema (CME) after central retinal vein occlusion (CRVO) with spectral-domain optical coherence tomography (SD-OCT). Methods Diagnosed with CRVO and CME by fundus fluorescein angiography, 50 eyes of 50 patients underwent SD-OCT. Early Treatment Diabetic Retinopathy Study (ETDRS) subfield identification was employed to measure and analyze macular thickness and cubic volume, which were then compared with measurements of contralateral eyes or sex-controlled and age-controlled ones. Results Compared with contralateral eyes and controlled eyes, CRVO-CME eyes had thicker macule in hill shape with peak at central area. The changes of cubic volume of subfields were in accordance with thickness changes. There were correlations between foveola thick- ness (FT) and best corrected visual acuity (BCVA in LogMAR), mean central area retinal thickness (FMcme) and BCVA, maximum central area retinal thickness (Fmax) and BCVA, and between cen- tral area retinal volume (FV) and BCVA. Conclusions Compared with control eyes, the macula of CRVO-CME eye is thicker and hill-like. SD-OCT is of great importance in precisely measuring the thickness and volume of macule with CRVO and CME, and providing evidence for the diagnosis and treatment of CME after CRVO.
