白内障超声乳化术中术后24h内突发严重全身合并症分析

摘　　要：目的总结白内障超声乳化手术术中及术后24h内突发严重全身合并症患者的临床特点及处理方法。方法回顾性临床研究。收集2010年1月到2013年12月在西安市第四医院眼科白内障组进行白内障超声乳化手术治疗的患者共15246例，分析所有从术中到术后24h内出现严重全身合并症患者的年龄、性别、发病时间、疾病类型及转归。结果共有12例出现术中及术后24h严重全身合并症，发生率为0．07％，平均年龄为（76．7±7．1）岁。其中男性6例，女性6例；有1例患者因术中出现严重全身合并症而暂停手术，给予急救处理，其余11例患者的全身合并症均在术后24h内出现。术中及术后24h严重全身合并症包括：脑出血、脑梗死、吸入性肺炎、急性心肌梗死、急性胆囊炎。所有出现严重全身合并症患者经过急救处理或相关专科处理，病情稳定后出院。完成手术的11例眼部病情稳定，术后视力均得到不同程度提高。结论白内障超声乳化手术术中及术后24h内可能发生全身严重合并症，眼科医师应注意观察，谨慎处理。Objective To summarize the clinical features and treatment of serious systemic compli- cations occurred during or postoperative within 24 hours of phacoemulsification, provide guidance to deal with such situations for ophthalmologists. Methods A retrospective clinical study. Collected 15246 cases who had phacaemulsification from January 2010 to December 2013 in Xi＇an NO.4 hospi- tal. Those patients who had occurred serious systemic complications during or postoperative within 24 hours of phacoemulsification was recorded. The patients＇ age, sex, time of onset, disease and outcome were analyzed. Results The ratio of serious systemic complications of our study was 0.07%, the aver- age age of those patients was 76.74-7.1 years, there were 6 males and 6 females; one of those patients had suspended operation because of serious systemic complications, other eleven patients occurred seri- ous systemic complications at postoperative within 24 hours of phacoemulsification. Serious systemic complications included： cerebral hemorrhage, cerebral infarction, acute myocardial infarction, aspiration pneumonia, acute cholecystitis. All those patients were in stable condition left hospital after emergency treatment or related specialist treatment. The visual acuity was restored to varying degrees of those 11 patients who completed surgery. Conclusions Serious systemic complications may occur during orpostoperative within 24 hours of phacoemulsification, ophthalmologist should observe and dispose carefully.
