摘 要:目的探讨链式激活的免疫细胞(Capri)治疗恶性积液的效果。方法分别观察试验组、化疗组患者恶性积液治疗的有效性和总生存期,应用流式细胞术和RT-PCR法检测2组患者治疗前后不同时间的T淋巴细胞亚群及CK19m RNA的变化。以健康体检者为对照组。结果试验组积液的控制情况和生活质量明显优于化疗组(χ2=7.810,P<0.05;χ2=5.185,P<0.05)。2组患者治疗前CD3+、CD4+、NK细胞含量明显低于对照组(P<0.05),治疗后试验组明显升高,与化疗组同期相比差异有统计学意义(P<0.05),与对照组相比差异无统计学意义(P>0.05)。2组患者治疗前CD8+含量明显高于对照组(P<0.05)。治疗后明显下降,与治疗前相比差异有统计学意义(P<0.05),与对照组相比差异无统计学意义(P>0.05),试验组与化疗组同期相比差异无统计学意义(P>0.05)。2组治疗前CK19m RNA基因表达均显著增强,与对照组相比差异有统计学意义(P<0.05)。治疗后试验组表达显著降低,与治疗前相比差异有统计学意义(P<0.05),与化疗组同期相比差异均有统计学意义(P<0.05)。结论恶性积液患者可以应用Capri细胞治疗,Capri细胞通过改善患者的免疫功能,增加患者NK细胞的数量,提高患者抗肿瘤的能力,有利于恶性积液患者的治疗及预后。Objective The aim of this study was to explore the curative effect on the chain activation of immune cells for treating malignant effusion. Methods By comparing the differences of lymphocyte subsets and CK19 mRNA levels before and after cascade primed immune cells(Capri) treatment and intrathracic chemotherapy of malignant effusion, we tried to discover the possible therapeutic mechanism. In total, 82 cases were randomized into 2 groups: chemotherapy group(41case), Capri immune cell group(41 case). The effective rate of control malignant effusion and score of KPS were observed and compared. The changes of lymphocyte subsets and CK19 mRNA levels were detected by FCM and RT-PCR, respectively, before and after the different treatments, and another 82 health adults were controlled. Results For efficient control of malignant effusion, it was significantly different between Capri treatment group and chemotherapy group(χ^2=7.810, P〈0.05). For score of KPS, Capri treatment group was statistically significant from the chemotherapy group(χ^2=5.185, P〈0.05). As compared with control group, CD3^+, CD4^+and NK T lymphocytes in peripheral blood decreased apparently in malignant effusion patients(P〈0.05), and CD8^+T lymphocytes significantly increased(P〈0.05). After treatment, CD3^+, CD4^+and NK T lymphocytes cells significantly increased, and there were significantly statistical differences between the Capri treatment group and chemotherapy group(P〈0.05). There were no significantly statistical differences between the Capri treatment group and control group(P〉0.05). For CD8^+T lymphocytes before treatment, the two groups of cases were significantly higher than control group(P〈0.05). There were no significantly statistical differences between the Capri treatment group and chemotherapy group and control group(P 0.05).
