TACE联合RFA治疗原发性肝癌的疗效观察

摘　　要：目的分析TACE联合RFA治疗原发性肝癌的疗效。方法将90例原发性肝癌患者随机分为两组,对照组和观察组,每组45例。对组采用TACE治疗,观察组则采用TACE联合RFA治疗。比较两组疗效差异及治疗前后Child-Pugh分级差异。比较治疗前后两组患者间血清中肝功能相关指标ALT、AST、TBIL、ALB、γ-GT、AFP水平差异及血清肿瘤标志物AFP、CEA、CA19-9、CA125和VEGF水平差异。结果观察组患者完全缓解率、部分缓解率及总有效率显著高于对照组,进展率则显著低于对照组,P<0.05;观察组术后1、3年生存率显著高于对照组,P<0.05。治疗后第2、4个月两组间Child-Pugh分级有显著差异(P<0.05),观察组肝功能优于对照组;观察组ALT、AST、TBIL、γ-GT、AFP显著低于对照组,ALB则显著高于对照组,P<0.05。观察组患者血清肿瘤标志物AFP、CA19-9、CA125和VEGF水平显著低于对照组,P<0.05。结论 TACE联合RFA治疗原发性肝癌能有效改善患者的肝功能,下调肿瘤标志物水平,提高肿瘤的缓解率,延长生存期,疗效优于TACE单用。Objective To observer the efficacy of TACE combined with RFA for patients with primary liver cancer（PLC）. Methods 90 cases of patients with PLC were randomly divided into two groups, control group and observation group, each group has 45 cases. Patients in control group were took TACE treatment, those in observation group were used TACE combined with RFA therapy. Then the difference of efficacy and Child-Pugh grading of two groups were compared. And liver function related indicators ALT, AST, TBIL, ALB, γ-GT, AFP and serum tumor markers AFP, CEA, CA19-9, CA125 and VEGF of two group were also compared. Results The complete remission rate, partial response rate and total efficiency of observation group were all higher than control group, progression rate was significantly lower than control group, and the survival rate was significantly higher than control group after 1, 3 years, P〈0.05.2, 4 months after treatment, there was significantly difference of Child-Pugh classification between the two groups, the liver fimction of observation group was better than the control group; and the serum levels of ALT, AST, TBIL, γ-GT, AFP of observation group were all lower than control group, while the ALB was higher than control group, P〈0.05; and serum levels of tumor markers AFP, CEA, CA19-9, CA125 and VEGF observation group were all lower than control group, P〈0.05. Conclusion TACE combined with RFA can improve liver function in patients with PLC, down the levels of tumor markers, improve tumor response rate, the efficacy was better than TACE alone.
