摘  要:目的探讨小型猪冠状静脉窦(CS)导管放置的方法。方法16头雄性中华小型猪,4头用于预实验,12头用于正式实验。通过观察小型猪心脏的解剖结构,以及在X线下不同投照角度的CS开口和走形位置,摸索经股静脉途径放置CS导管的方法,并对操作者所受放射剂量进行评估。结果正式实验中,操作者通过股静脉将导丝、导管送入右心房,退出导丝让导管自然打弯,轻轻按顺时针方向转动导管使导管头端指向左侧可将导管顺利送入CS口内。手术的操作时间较短,分离血管的平均时间仅为(15.5±5.8)min,曝光时间仅为(112±20)s。放置导管成功率为12/12。仅1头(1/12)猪股静脉拔鞘压迫后出现血肿,未发生冠状静脉损伤穿孔、心包填塞等并发症。操作人员的人均所受放射剂量仅为(1.56±0.32)μgay。结论这种简单、易行、安全、可靠的CS导管放置方法能为用猪作为心脏研究动物模型的科研工作者们提供帮助。Objective To explore how to insert a catheter into coronary sinus through the femoral vein in miniature swine. Methods Anatomic structure of the swine heart, coronary sinus ostium and femoral vein at different angles under X-ray were observed. And the radiation dose received by operators was evaluated. Results The operator put the guide wire and catheter into the right atrium through femoral vein, and then pull the guide wire out of the catheter which could bend spontaneously. Then the operator gently rotated the catheter clockwise to the left and the catheter could go into the coronary sinus ostium smoothly. The operation time was shorter: with the time for blood vessel separation being only (15.5± 5.8) min and the exposure time being only (112± 20) s . The operation was successful in'all cases. Only one pig developed hematoma after distraction of the sheath. No serious complications such as perforation of coronary vein or pericardial tamponade were noticed. The radiological dose was only ( 1.56 ±0.32) pgay per operator. Conclusion This method is simple, safe and reliable, which can offer help for scientific researchers using swine as animal models of heart study.
