高度近视白内障超声乳化人工晶状体植入术的效果

摘　　要：目的 评价高度近视合并白内障行超声乳化吸出及折叠人工晶状体植术的临床效果.方法 对36例（48眼）高度近视合并白内障行透明角膜切口超声乳化吸出及折叠人工晶状体植入术.眼轴长度（26.78～34.56）mm,平均（27.43±2.80） mm,其中眼轴≤30 nm者36眼;眼轴＞30mm者12眼.观察术后视力及术中术后并发症.结果 术中悬韧带离断1眼,后囊破裂2眼,经处理均植入折叠人工晶状体于囊袋内.眼轴≤30 mm的36跟中术后视力≥0.3者30眼（83.3％）;眼轴＞30 mm的12眼中术后视力≥0.3者5眼（41.7％）.结论 透明角膜切口超声乳化吸出及折叠人工晶状体植入术,适合于高度近视合并白内障.但眼轴＞ 30 mm者术后视力恢复较差,与眼轴长度及黄斑功能有关.Objective To evaluate the clinical effects of phacoemulsification and foldable intraocular lens implantation for high myopia with cataract.Methods Phacoemulsification and foldable intraocular lens implantation through a clear corneal incision was performed on 48 eyes of 36 cases with high myopia and cataract.Among these cases,there were 30 eyes with axial lengths equal or less than 30 mm,18 eyes with axial lengths more than 30 mm,and the average was （27.43 ± 2.80） mm.The postoperative vision,intraoperative and postoperativecomplications were observed.Results Intraoperative suspensory ligament rupture happened in 1 eye,posterior capsular tear happened in 2 eyes,and the intraocular lens were all implanted into the capsular bag after the complications were dealt with.Among the 36 eyes of axial length equal or less than 30 mm,the vision was better than 0.3 in 30 eyes （83.3%）,Among the 12 eyes of axial length more than 30 mm,the vision was better than 0.3 in 5 eyes （41.7%）.Conclusion Surgery of clear corneal incision phacoemulsification and foldable intraocular lens implantation is effective for high myopia with cataract,but the postoperative vision in patients with axial lengths more than 30 mm is poor which is related to axial length and macular function.
