林西医院白内障手术202眼临床效果分析

摘　　要：目的 观察小切口非超声乳化摘出术及超声乳化吸出术治疗老年性白内障的临床效果.方法 作者所在的林西医院2011年1月至2013年12月期间收治的老年性白内障156例（202眼）,随机分为两组：A组,白内障小切口非超声乳化摘出术组78例（105眼）;B组,晶状体超声乳化吸出术组78例（97眼）.比较两组术后1、7、30 d时的视力、角膜散光度、术中及术后常见并发症的情况.结果 两组裸眼视力在术后1d呈现出明显的差异,A组视力改善明显优于B组（x2 =4.150,P=0.042）,而术后7d和30 d的裸眼视力改善情况两组间差异无统计学意义.A组术后ld的角膜散光度明显高于B组（t=2.345,P=0.02）,而术后7d和30 d比较两组间差异无统计学意义（t=0.676,0.214,P=0.510,0.816）.两组患者术后并发症发生情况比较差异无统计学意义.结论 小切口非超声乳化摘出术治疗老年性白内障疗效好,操作简单,术者学习曲线短,费用低,宜于在基层医院推广.Objective To observe the efficacy of small incision non-phacoemulsification extraction of cataract for the senile cataract.Methods Two hundred and two eyes of 156 cases with senile cataract in our hospital between January 2011 and December 2011 were randomly divided into two groups.Seventy-eight cases （105 eyes） in group A underwent the small incision manual cataract extraction,and 78 cases （97eyes） in group B received phacoemulsification surgery.The visual acuity,corneal astigmatism and the complication 1,7 and 30 days after surgery were compared between two groups.Results The vision of patients in group A 1 day after surgery was better than that of patients in group B （x2 =4.150,P =0.042）,and the difference was statistically significant.However,the difference in vision of patients between 7 and 30 days after surgery was not statistically significant.The postoperative corneal astigmatism of patients in group A was significantly higher than that of patients in group B （t =2.345,P =0.02）,and between 7 and 30 days after the operation the difference was not statistically significant （t =0.676,0.214,P =0.510,0.816）.The difference in complications between two groups had no statistical significance.Conclusion Small incision non-phacoemulsification for senile cataract is effective,simple,easy to learn and cheap.It is recommended to perform in primary hospitals.
