生存素蛋白在弥漫性大B细胞淋巴瘤中的表达及意义

摘　　要：目的:探讨生存素(survivin)蛋白在弥漫性大B细胞淋巴瘤(DLBCL)中的表达及其与细胞增殖及凋亡的关系。方法:采用免疫组织化学的方法,对48例弥漫性大B细胞淋巴瘤(DLBCL)和10例淋巴结反应性增生(RHL)组织中生存素蛋白、caspase-3、ki-67的表达进行检测,分析生存素蛋白表达与caspase-3及ki-67增殖指数的关系。结果:生存素蛋白在10例RHL中均无表达,48例DLBCL中的阳性表达率为79.2%(38/48),其在DLBCL中的阳性表达率明显高于在RHL中的表达(P<0.05);caspase-3在48例DLBCL和10例RHL中的阳性表达率分别为50%(24/48)和80%(8/10)(P>0.05),两者无明显差别。生存素蛋白阳性表达的DLBCL中ki-67增殖指数(80.66±11.04)%明显高于阴性表达组(62.00±6.32)%(P<0.05);生存素蛋白表达与caspase-3无明显相关(P<0.05)。结论:生存素蛋白在DLBCL中表达较高,survivin可能通过促进肿瘤细胞的增殖,从而在DLBCL的发生发展中起到重要的作用。Objective： To investigate the expression of survivin protein in diffuse large B cell lymphoma（ DLBCL and its relationship with tumor cell apoptosis and proliferation. Methods： Immunohistostaining was performed to detect the expression of survivin,caspase-3 and ki-67 in the samples from 48 DLBCL patients and 10 with reactive hyperplasia of lymph node（ RHL）,and the proliferation index was analyzed between survivin protein expression and tumor cell apoptosis and tumor cell proliferation. Results： Survivin expression was free in the tissues of RHL,whereas accounted for 79. 2%（ 38 /48） in DLBCL（ P〈0. 05）. The expression of caspase-3 in DLBCL and RHL were 50%（ 24 /48） and 80%（ 8 /10）,respectively. The difference was not significant（ P〉0. 05）. Positive survivin expression was 50%（ 24 /48） in the 48 DLBCL cases and 80%（ 8 /10） in the 10 RHL（ P〉0. 05）,and the proliferation index for ki-67 was significantly higher than that in survivin protein negative cases（ 80. 66 ± 11. 04 vs. 62. 00 ± 6. 32）（ P〈0. 05）. Survivin expression was not associate with caspase-3 expression in DLBCL（ P〉0. 05）. Conclusion： Higher expression of survivin protein in DLBCL may lead to tumor cell proliferation and function a lot in the development of DLBCL.
