45例周围型肺癌的X线与CT影像学分析

摘　　要：目的:探讨周围型肺癌的影像学表现特征,对比分析X线平片与CT扫描在周围型肺癌中的应用价值。方法:选取45例经病理证实为周围型肺癌患者的X线平片、CT扫描特征进行回顾性分析。结果:X线平片显示病灶边缘为分叶征31例,细短毛刺征23例,病灶内部为空泡征7例,空气支气管征8例,病灶周围为胸膜凹陷征19例,血管集束征13例。CT检查显示病灶边缘为分叶征37例,细短毛刺征33例,病灶内部为空泡征17例,空气支气管征21例,病灶周围为胸膜凹陷征25例,血管集束征24例。X线平片的诊断准确率为(35/45)78%,CT的诊断准确率为(42/45)93%。结论:分叶征、细短毛刺征、空泡征、空气支气管征、胸膜凹陷征、血管集束征是周围型肺癌的重要征象,X线平片与CT扫描是临床诊断周围型肺癌的两种重要手段,CT扫描能更清晰地显示周围型肺癌的基本征象,能够有效提高周围型肺癌的早期诊断率。Objective To investigate the imaging features of peripheral lung cancer, contrast analysis the application value of X - ray plain film and CT scan in the peripheral lung cancer. Method 45 patients confirmed peripheral lung cancer by pathological were collected. The characteristic of X - ray plain film and CT scan was analyzed retrospectively. Results X - ray results showed 31 cases with the lesion edge for lobulation,23 cases with fine short speculation,7 cases with lesions internal to the vacuole sign ,8 cases with air bronchogram sign, 19 cases with the lesions around the pleural indentation sign and 13 cases with vascular convergence sign. CT examination showed 37 cases with lesionedge for lobulation sign,33 cases with fine short spiculation sign, 17 cases with lesions internal to the vacuole sign,21 cases with air bron- chogram sign,25 cases with the lesions around the pleural indentation sign and 24 cases with vascular convergence sign. Therefore,X -ray di- agnosis accurate rate of was 78% （35/45） and the diagnostic accuracy of CT was 93% （42/45）. Conclusion Lobulation ,fine short spicula- tion, vacuole sign, air bronchogram,pleural indentation and vascular convergence are an important sign of peripheral lung cancer. X -ray and CT scanning are two kinds of important means for clinical diagnosis of peripheral lung cancer. CT scan can show more clearly the basic fea- tures of operipheral lung cancer,it can effectively improve the rate of early diagnosis of peripheral lung cancer. Key Words：
