虹膜夹持型人工晶体植入联合虹膜周切术治疗无晶体眼的临床观察

摘　　要：目的：探讨并总结虹膜夹持型人工晶体（irisclipintraocularlensIOL）植入联合虹膜周切术治疗无晶体眼的临床疗效。方法：对15例15眼无后囊或少后囊存在的无晶体眼进行虹膜夹持型人工晶体植入联合虹膜周切术。术后1天、1周、1月、3月随访，观察术前术后裸眼视力（visualacuity，VA）、最佳矫正视力（bestcorrectedVisualacuity，BCVA）、角膜透明性、眼压（intraocularpressure，10P）、角膜内皮细胞计数（cornealendothelialcells，CECs）及术后并发症情况。结果：术后3月，术后VA较术前显著提高；术前、术后BCVA无显著变化；术后3月角膜均恢复透明（除一眼术前即已因外伤继发角膜瘢翳而混浊），术后3月IOP平稳正常，CECs较术前无或仅有少量减少，无一例出现人工晶状体移位、继发性青光眼、脉络膜脱离、出血、炎症渗出等并发症。结论：虹膜夹持型人工晶体植入联合虹膜周切术治疗无晶体眼，具有屈光矫正效果好、稳定性高、并发症少、操作相对简-g、组织损伤小等优点，疗效理想，值得推广。Objective to evaluate the therapeutic effect and complications of artisan iris clip intraocular lens （iols） implantation for various aphakia ; Methods operated iris clip intraocular lens （iols） implantion on 15 cases（15 eyes） aphakia with no or few posterior capsular ,the postoperative visual outcomes（VA and BCVA）, intraocular pressure0OP）, corneal endothelial cells（CECs） and complications were analyzed in 3 months follow -up ; Results the postoperative visual acuity is significantly improved（preoperative 0.05±0.02, postoperative 0.43±0.15, t=4.58, P〈0.05）, while the BCVA had no significant changes（preoperative 0.43±0.15, postoperative 0.45±0.17, t=1.72, P〉0.05） , the postoperative lOP keep normal and stable （14.72±2.53） mmHg, there had no significant less in postoperative CECs （postoperative CECs was （1683.76±506.27） /ram2, preoperative CECs was （1850.97±532.42） /mm2）, all corneals recoverd transparent except I who has corneal scar because of corneal injury, all cases had no obvious postoperative complications such as IOL displacement, secondary glaucoma, choroid or retinal detachement, bleeding or inflammation effusion, etc. Conclusion the therapeutic effect of iris clip intraocular lens （ iols） implantion for aphakia is satisfying and worth promoting.
