腹膜假粘液瘤围手术期麻醉管理分析

摘　　要：目的：总结我院近6年来腹膜假粘液瘤手术麻醉的临床经验。方法回顾性分析2008年10月1日~2014年10月1日在我院择期行腹膜假粘液瘤手术患者150例临床资料，其中2008年10月1日~2010年12月31日的62例作为A组，2011年1月1日~2014年10月1日的88例作为B组。对比分析两组术前准备情况，术后ICU停留时间，术后住院时间及术后30d并发症发生率。结果与A 组比较，B 组手术准备时间缩短8.6d，术后 ICU 停留时间缩短4.9d，术后住院时间缩短6.9d，术后30d并发症中肠瘘、肠梗阻、肺部感染与术后出血的发生率显著降低。两组静脉血栓及死亡的发生率无显著性差异。结论通过优化围手术期治疗策略，可减少手术失血量和异体血输注量，改善粘液瘤手术围术期的预后，对进一步优化术中麻醉管理具有指导意义。Objective To summarize the clinical experience of operation anesthesia for pseudomyxoma peritonei in our hospital in recent six years. Methods Retrospectively analysed the clinical data of 150 cases underwent peritoneal pseu-domyxoma operation from Oct 1, 2008 to Oct 1, 2014. 62 cases from Oct 1, 2008 to Dec 31, 2010 were treated as group A, while 88 cases from Jan 1, 2011 to Oct 1, 2014 were treated as group B. Compared and analysed the preoperative preparation, ICU residence time and hospitalization time after operation, the incidence rate of postoperative complications at 30 days after operation between the two groups. Results Compared with group A, the preoperative preparation time was shortened by 8.6 days, ICU residence time was shortened by 4.9 days, hospitalization time was shortened by 6.9 days, the incidence rate of in-testinal fistula, ileus, pneumonia, postoperative bleeding at 30 days after operation was lower in group B. There was no signifi-cant difference in the incidence rate of deep venous thrombosis （DVT） and the mortality between the two groups. Conclu-sion Optimizing perioperative treatment strategies can reduce operative blood loss and the amount of allogeneic blood transfu-sion and improve the prognosis of pseudomyxoma peritonei,it has guiding significance for the optimization of anesthesia manage-ment in operation.
