摘 要:目的 探讨经胃镜植入化疗粒子行肿瘤局部化疗联合树突状细胞-多因子诱导的杀伤细胞(DC-CIK)免疫细胞输注法对隆起型食管癌患者的临床疗效。方法 选取2013年10月-2015年4月间于该院证实为隆起型食管癌的患者共200例按随机数字法分为化疗粒子植入联合DC-CIK免疫细胞输注法组100例(联合组)及单独化疗粒子植入组100例(对照组)。联合组患者给予两种方法联合治疗,对照组患者只给予内镜下化疗粒子植入法。1~2疗程后对两组患者的症状改善情况、内镜下肿瘤缩小程度以及1年生存率数据进行提取统计。两组基础临床资料差异无显著性(〉0.05),具有可比性。结果 联合组症状改善患者(94%)高于对照组(80%),两组数据经χ2检验差异有显著性(χ2=11.493,=0.003);联合治疗组中有58例达到完全缓解,而对照组仅有37例,两组数据经χ2检验差异有显著性(χ2=8.842,=0.000);联合组的1年生存率(71%)高于对照组(57%),差异有显著性(χ2=4.253,=0.039)。结论 经胃镜植入化疗粒子行肿瘤局部化疗和DC-CIK免疫细胞输注法治疗隆起型食管癌的效果优于单用植入化疗粒子法,具有简便、副作用小且易于操作等特点。[ Objective ] To explore the clinical efficacy of endoscopic implantation with chemotherapeutic slowrelease combined with dendritic eells-cytokine induced killer (DC-CIK) immune cells infusion for patients with obstructive esophageal cancers. [Methods] A total of 200 patients diagnosed with esophageal carcinoma in our hospital between October 2015 to April 2013 were divided into two groups, 100 case each , according to the random numbers. The combination group was given the chemotherapeutic slow-release combined with DC-CIK immune cells infusion and the control group was only treated by the chemotherapeutic slow-release. After 1 -2 treatment courses, the data of the symptom improvements, the extent of endoscopic tumor and one-year survival rate in patients of two groups were extracted to make statistics. The basic clinical data between two groups were no statistically differences (P 〉0.05). [ Results ] The symptom improvement rate of combination group (94%) was higher than that in the control group (80%), there was statistically significant difference between two groups by chi-square methods (x2=11.493, P = 0.003); A total 58 cases in combination group achieved complete remission while there were only 37 cases in thecontrol group, and the differences had statistically significance (X2=8.842, P =0.000). The one-year survival rate (71%) of the combination group was statistically higher than the control group (57%) by the chi-square methods (X2= 4.253, P =0.039). [Conclusion] The effect of the therapy combined endoscopic implantation with chemotherapeutic slow-release with DC-CIK immune cells infusion that had the characteristics of simple, limited side effects, easy to operate which could be used as a feasible treatment method for esophageal cancers.
