摘  要:目的评价三种不同干预方式对口腔固定正畸治疗中牙釉质脱矿的影响。方法经签字同意进入实验的正畸患者60例,随机分为A、B、C三组,每组20名。A组:单纯口腔卫生措施。B组:在A组的基础上使用氟化泡沫。C组:在A组的基础上使用酪蛋白磷酸多肽-无定形磷酸钙。三组患者从治疗开始每月都使用Canon60D专业相机拍一次牙牙合照片,共拍摄13次,观察时间为一年。临床检测指标为牙面是否出现新增的白垩色斑或齲洞,计算牙釉质脱矿率。采用SPSS15.0软件包,数值变量资料用秩和检验,分类数据用x2检验。结果从12次牙釉质脱矿变化看,A、B、C三组的牙釉质脱矿率都随着时间的延长而增;三种不同处理方式脱矿率比较;B组与A组比较脱矿率差异有统计学意义(P<0.05),C组与A组比较脱矿率差异有统计学意义(P<0.05),B组与C组比较脱矿率差异无统计学意义(P>0.05)。结论1固定正畸所导致的牙釉质脱矿的发生率和严重程度与正畸时间呈正相关。2在接受固定矫治器治疗的正畸患者,定期使用氟化泡沫和GC护牙素均能够有效减少釉质脱矿的发生。Objective For preventing enamel demineralization in the fixed orthodontic process, the aim of this study was to observe the clinical results of using Casein phosphopeptide- amorphic calcium phosphate(CPP-ACP). Methods 60 volunteers who were undergoing fixed orthodontics participated in this study ,they were randomly divided into 3 groups ,20 volunteers in each group. Group A was control group, received routine dental health care. Group B appended fluoride foam based on Group A while receiving routine dental health care. Group C appended CPP-ACP based on Group A. From they started to receive the treatments, their occlusion pictures were taken per month,totally 13 times in a year. The clinical test index was to observe whether new plaque or caries appeared on the tooth surfaces, meanwhile the enamel demineralization index(EDI) was calculated. The SPSS 15.0 pack program was used for statistical analysis. Rank sum test was used to determine the numerical variable data, categorical data was de- termined by using the chi-square test. Results From the change of enamel demineralization in 12 times in 3 groups,the EDI was increasing as time went on;The mean ± SD of EDI in 3 groups:compared with B, A was higher, had statistical significance (P 〈 0. 05 ) ; meanwhile, A was higher than C, also had statistical significance ( P 〈 0.05 ) ; compared B with C, there was no statistical significance( P 〉 0. 05 ). Conclusion ①The EDI and enamel demineralization damage degree had a positive correlation to the time spent in fixed appliance;②The volunteers who received Fluoride foam treatment each time, their EDI reduced effectively;To use CPP-ACP regularly was the effective and safety way to prevent enamel demineralization among volunteers.
