摘 要:目的:运用代谢组学技术对2型糖尿病气阴两虚证患者尿液中内源性小分子代谢物进行分析,寻找2型糖尿病气阴两虚证的特征性代谢产物。方法:收集2型糖尿病气阴两虚证患者30例,2型糖尿病非气阴两虚证患者25例,以及30例健康人作为正常对照组。以气相色谱-质谱联用(GC-MS)技术对各组受试者尿液样本进行代谢图谱分析,对潜在的生物标记物进行鉴定,探索2型糖尿病气阴两虚证的潜在代谢标记物。结果:与正常对照组相比较,2型糖尿病组D-葡萄糖、D-葡萄糖酸、甘氨酸、2,3,4-三羟基丁酸、肌醇、尿素含量均升高(P<0.05或P<0.01),马尿酸含量降低(P<0.01)。气阴组与非气阴组比较,甘氨酸、d-半乳糖含量升高,木糖醇、马尿酸含量降低,差异具有统计学意义(P<0.05或P<0.01)。结论:,2型糖尿病气阴两虚证患者与非气阴两虚证患者的尿液代谢图谱存在差异,对分类有贡献的潜在代谢标记物可能是2型糖尿病气阴两虚证中医辨证分型的物质基础。Objective: In order to find out potential metabolic markers in urinary of type 2 diabetes mellitus' s patients with syndrome of both Qi- yin deficiency by metabolic method. Methods : 3 0 cases of type 2 diabetes mellitus 's patients with both Qi- Yin deficiency syndrome,25 cases of type 2 diabetes mellitus' s patients without Qi- Yin deficiency syndrome and 30 healthy persons were involved in the research. Gas Chromatography- Mass Spectrometry( GC- MS) was used to analyze metabolic characteristic of the urine samples. The potential biomarkers were identified to explore the potential metabolism markers of type 2 diabetes mellitus' s patients with both Qi- Yin deficiency syndrome. Results: Compared with the normal group,the contents of D- glucose,D- gluconic acid,glycine,2,3,4- trihydroxybutyric acid,inositol and urea were increased in type 2 diabetes mellitus' s group( P〈0. 05 or P〈0. 01). The contents of hippuric acid was also decreased significantly( P〈0. 01). Compared with type 2 diabetes mellitus' s patients without Qi- Yin deficiency syndrome,the contents of glycine and d- galactose were increased in type 2 diabetes mellitus' s patients with Qi- Yin deficiency syndrome( P〈0. 05 or P〈0. 01),and the contents of hippuric acid and xylitol were decreased significantly( P〈0. 05 or P〈0. 01). Conclusions: The difference is significant in the urine metabolism between the patients of type 2 diabetes mellitus with Qi- Yin deficiency syndrome and who without Qi- Yin deficiency syndrome. The substance contributing to classification may be the potential metabolic marker for TCM pattern identification of type 2 diabetes mellitus with Qi-Yin deficiency syndrome.
