入院时血糖水平与脑梗死静脉溶栓患者预后的相关性研究

摘　　要：目的探讨入院时血糖水平与急性脑梗死患者静脉溶栓预后的关联。方法本项研究为回顾性研究,共纳入脑梗死溶栓患者97例,根据入院时血糖水平分为二组：入院时高血糖组（血糖≥7.8mmol/L）及血糖正常组（血糖〈7.8mmol/L）。收集二组患者的基线资料,并应用改良的Rankin量表（mRS）评价预后,通过Logistic回归分析研究入院时血糖与静脉溶栓预后的相关性。结果入院时高血糖组患者的入院时NIHSS评分、高脂血症及糖尿病比率高于血糖正常组,差异有统计学意义（P〈0.05）。入院时高血糖组90d良好预后的患者比例较血糖正常组低,分别为46.15%、53.44%,差异具有统计学意义（P〈0.05）。Logistic回归分析显示入院时高血糖（OR=2.893,95%CI 1.121-6.921,P=0.022）及糖尿病（OR=2.615,95%CI1.032-3.997,P=0.029）与预后不良相关。结论急性缺血性卒中患者入院时血糖水平的升高与静脉溶栓治疗预后不良相关。Objective To explore the relation between admission blood glucose levels and prognosis of acute ischemic stroke patients treated with intravenous thrombolysis. Methods The research is a retrospective study. Totally 97 thrombolytic patients were included and divided into two groups according to admission blood glucose levels： admission hyperglycemia group（blood glucose ≥7. 8 mmol/L} and normal blood glucose group（blood glucose 〈7.8mmol/L）. Baseline characteristics were collected and prognosis was evaluated by modified Rankin scale. The relativity of admission blood glucose levels and prognosis after intravenous thrombolysis was analyzed by logistic regression model. Results Admission hyperglycemia patients on admission NIHSS score, hyperlipidemia and diabetes rates were higher than normal blood glucose group, with a statistical significance difference （P〈0.05）. The proportion of 90d good prognosis of admis- sion hyperglycemia group was lower than those of the normal blood sugar group （ P〈0, 05）, and they were 46.15% and 53.44% respectively. Logistic regression analysis showed that the admission hyperglycemia （OR=2.893, 95% CI： 1.121-6. 921, P=0.022） and diabetes （OR=2.615, 95% CI： 1.032-3.997,P = 0. 029 ） were associated with a poor prognosis. Conclusion Patient with acute ischemic stroke with elevated glucose levels and thrombolytic therapy is associated with poor prognosis.
