摘　　要：目的探讨不同方位角膜切口对高龄白内障术后泪膜和眼表的影响。方法选取2010年7月-2014年6月本院高龄（≥80岁）白内障超声乳化患者120例（120眼）,随机分成2组：A组采用颞侧角膜切口行白内障超声乳化手术58例（58眼）;B组采用上方角膜切口行白内障超声乳化手术62例（62眼）。分别于术前3 d和术后3、7、30、90 d观察比较两组患者角膜荧光素染色（FL）评分、泪膜破裂时间（BUT）、基础泪液分泌试验（SⅠt）检查。结果A组患者FL评分在术后3、7、30 d明显增高,BUT、SⅠt检查术后3、7、30 d明显缩短,差异均有统计学意义（P〈0.05）;A组患者90 d的FL评分、BUT、SⅠt检查与B组比较,差异均无统计学意义（P〉0.05）。结论高龄白内障超声乳化手术中早期颞侧角膜切口较上方角膜切口患者的泪膜稳定性和眼表情况均有明显的影响,晚期则无明显影响。Objective To explore the influence of different corneal incision orientation on tear film and ocular surface after aged cataract surgeries. Methods 120 aged cataract patients （120 eyes） who were all over 80 years old in our hospital from July 2010 to June 2014 were treated by phacoemulsification were randomly divided into two groups.In group A,58 patients （58 eyes） were operated with phacoemulsification in temporal corneal incisions,while in group B,62 patients （62 eyes） were received with phacoemulsification in corneal incision above.Corneal fluorescence staining （FL）, breakup time of tear film（BUT）,and SchirmerⅠtest （SⅠt） in these patients were observed and compared 3 days before surgery and 3 days,7 days,30 days,90 days after surgery. Results 3 days,7 days, and 30 days after surgery, corneal fluorescence staining in group A was obviously increased respectively,BUT and SⅠt were greatly shortened,with statistical difference （P〈0.05）.90 days after surgery,there was no statistical difference in FL,BUT,and SⅠt after comparisons between two groups （P〉0.05）. Conclusion Early-stage temporal corneal incision has more obvious influences on stability of tear film and ocular surface compared with those by corneal incision above in aged phacoemulsification,while in late stage,no such influences occur.
