摘　　要：目的：系统评价尿膀胱癌抗原（urinarybladdercancer，UBC）检测对膀胱癌的诊断意义。方法：制定原始文献的纳入、排除标准及检索策略，检索Pubmed（1996年1月～2014年8月）、EMBase（1996年1月～2014年8月）、CNKI（1996年1月～2014年8月）、万方数据库（1996年1月～2014年8月）、维普数据库（1996年1月～2014年8月），收集UBC检测诊断膀胱癌的有关文献，用MetaDiSc1．4软件进行Meta分析。结果：共检索到相关研究文献53篇，对纳入符合标准的22篇文献进行Meta分析，共3371人。异质性检验提示无阈值效应，但存在其他原因导致的异质性。用随机效应模型进行Meta分析。UBC检测诊断膀胱癌的敏感度为0．673，特异度为0．827，阳性似然比为3．447，阴性似然比为0．365，诊断优势比为9．993，综合受试者工作特征曲线下面积为0．8271，Q。指数为0．7599。结论：UBC作为一种敏感度、特异性均较高的无创性尿膀胱肿瘤标志物，可以作为膀胱癌诊疗的重要辅助手段，可在今后的膀胱癌防治工作中发挥重要作用。Objective To systematic review of urinary bladder cancer（UBC） test in the diagnosis of gladder cancer. Methods： Pubmed （1996. 1-2014. 8）, EMBase （1996. 1-2014. 8）, CNKI （1996. 1-2014. 8）, WanFang （1996.1-2014.8）, cqvip （1996.1-2014.8） were searched for studies about UBC in the diagnosis of bladder cancer. The search strategy was made according to the Collaborative Review Group search strategy. Data were extracted by two reviewers using the designed extraction form. The software MetaDiScl. 4 was used to review management and data analysis. Results：In total, 53 relevant studies were founded, of which 22 were included, with 3371 pa- tients involved. Heterogeneity but threshold effect was found within these studies. A meta-analysis was performed using random effect model. Pooled accuracy indicators of sensitivity, specificity, positive likelihood ratio （LR＋） , negative likelihood ratio（LR-） and diagnostic odds ratio（DOR） of UBC were 0. 673, 0. 827, 3. 447, 0. 365, 9.99a, respectively. Area under curve（AUC） of SROC curve of UBC was 0. 8271, and Q-index was 0. 7599. Con- cliusions; UBC had a good accuracy performance in the the diagnosis of bladder cancer, and could be an available and important adjunct in the the diagnosis of bladder cancer.
