前列腺巨大恶性孤立性纤维瘤1例报告并文献复习

摘　　要：目的:探讨前列腺巨大恶性孤立性纤维瘤的临床表现、影像学表现、病理特点及诊疗方法。方法:回顾性分析我院收治的1例前列腺巨大恶性孤立性纤维瘤患者的临床资料,并结合国内外文献进行分析。结果:患者在全麻下行膀胱前列腺根治性切除术+双侧输尿管皮肤造口术。术中所见前列腺肿瘤大小约11.5cm×8cm×10cm,包膜尚完整,前壁与膀胱粘连紧密,后壁与直肠前间隙欠清。经病理及免疫组织化学检查确诊。术后患者恢复良好,随访3个月未见肿瘤复发及转移。结论:前列腺巨大恶性孤立性纤维瘤极其罕见,临床表现无明显特异性,确诊仍需病理检查和免疫组织化学检查。其治疗方法可根据患者临床情况决定。Objective： To investigate the clinical manifestations, radiological and pathologic features and treat- ments of huge malignant solitary fibrous tumor of the prostate. Method： We summarized the clinical data of one patient with huge solitary malignant fibroma of prostate in our department, and reviewed the related literature. Result： A 59-year-old man underwent radical cysto-prostatectomy and bilateral ureteral cutaneous ureterostomy under general anesthesia. The tumor was seen 11.5 cruX8 crux 10 cm. The wall of the tumor was complete. A close adhesion between the anterior wall of the tumor and bladder was found, and the gap between the posterior wall of the tumor and the anterior wall of the rectum couldn＇t be seen clearly. Pathological and immunohistoehem- istry results confirmed the diagnosis of malignant solitary fibroma of prostate. The patient recovered well after the operation, and no recurrence or metastasis was seen over the 3-month follow-up period. Conelusion~ Huge malig- nant solitary fibroma of prostate is rare with no specific clinical manifestation. Confirmed diagnosis needs the evi- dence from pathologic examination and immunohistoehemistry. Choices of treatments base on patients＂ clinical da- ta.
