摘  要:目的探讨血清胸腺嘧啶核苷激酶1(TK1)、癌胚抗原(CEA)和糖类抗原153(CA153)水平与进展期乳腺癌化疗疗效及患者预后的关系。方法检测进展期乳腺癌患者血清TK1、CEA和CA153水平,化疗2个周期后对所有患者进行疗效评定,随访24个月。结果共入组85例进展期乳腺癌患者,随访结束疗效评定达完全缓解(CR)28例,部分缓解(PR)22例,疾病稳定(SD)18例,疾病进展(PD)17例。血清TK1、CEA和CA153指标联合预测患者发生PD的敏感度为60.0%,特异度为88.6%,其敏感度高于使用单项指标预测,特异度低于单独使用TK1,但高于单独使用CEA或CA153。结论 TK1、CEA和CA153是进展期乳腺癌有效的化疗疗效预测指标,联合检测可提高预测患者发生PD的灵敏度。Objective To explore the clinical value of combined detection of serum marks thymi- dine kinase 1 ( TK1 ) , carcinoembryonic antigen (CEA) and carbohydrate 153 ( CA153 ) for the efficacy and prognostic significance of chemotherapy in advanced breast carcinoma. Methods The levels of TK1, CEA and CA153 were detected in patients with advanced breast carcinoma. After at least two cycles of chemotherapy, the response rate was evaluated and each patient was followed up for two years. Results Eighty-five cases were available for therapeutic effect evaluation, there were 28 cases of complete response (CR) , 22 cases of partial response (PR) , 18 cases of stable disease (SD) and 17 cases of progressive dis- ease (PD). The sensitivity of combined detection of TK1, CEA and CA153 was 60.0% to forecast patients with PD, which is higher than the use of individual tumor marker; and the specificity of combined detection was 88. 6%, lower than TK1 but higher than CEA or CA153. Conclusion Combined detection of serum TK1, CEA and CA153 can provide important value for prognosis and monitor disease progress of advanced breast carcinoma.
