骨转移癌患者发生下肢深静脉血栓的相关因素分析

摘　　要：目的探讨骨转移癌患者发生下肢深静脉血栓的相关因素。方法对112例骨转移癌患者进行彩色多普勒超声诊断,并对患者的年龄、性别、血型、卧床时间等影响患者发生下肢深静脉血栓的相关因素进行单因素和多因素分析。结果 112例骨转移癌患者中,30例患者发生下肢深静脉血栓,发生率为26.8%,其中男性17例,女性13例;有症状11例,无症状19例;左下肢5例,右下肢18例,7例为双下肢;4例为双下肢全深静脉血栓,26例为小腿肌间血栓;单因素分析显示,血栓形成与患者的血型、卧床时间、病理性骨折、病灶数目、红细胞压积和纤维蛋白原水平有关,差异均有统计学意义(均P<0.05);多因素分析结果显示,患者的卧床时间、是否伴有病理性骨折和纤维蛋白原水平是影响患者发生下肢深静脉血栓的独立因素,差异均有统计学意义(均P<0.05)。结论导致骨转移癌患者发生下肢深静脉血栓的因素是患者的卧床时间、是否有病理性骨折和纤维蛋白原水平。Objective To explore the factors of deep venous thrombosis of lower limbs in cancer patients with bone metastases. Methods A total of 112 cases of cancer patients with bone metastases were diagnosised by color Doppler ultrasound. Univariate and multivariate analyses were performed on the patient＇s age, gender, blood type, bed time related factors that affect the patients with deep venous thrombo- sis of lower limbs. Results Among 112 cases with bone metastases, 30 patients developed deep vein thrombosis, the rate was 26. 8% , including 17 males and 13 females. There are 11 cases of symptoms, no symptoms of 19 cases, 5 cases of left lower limb, right lower extremity in 18 cases, 7 cases of lower limbs, 4 cases for the whole lower extremity deep vein thrombosis, 26 cases of small leg thrombosis ; Univariate a- nalysis showed that thrombosis was related to learning, bed time, pathology fractures, number of lesions, hematoerit and fibrinogen levels （ P 〈 0. 05 ） ; Multivariate analysis showed that independent factors leading to patients with deep vein thrombosis were patient＇s bed time, whether accompanied by pathological fracture and fibrinogen levels （ P 〈 0.05 ）. Conclusions The factors leading to patients with deep vein thrombosis were patient＇s bed time, whether accompanied by pathological fracture and fibrinogen levels.
