宫颈癌患者血清CRP水平与临床病理特征和预后的关系

摘　　要：目的:探讨血清C-反应蛋白(CRP)水平与宫颈癌患者临床病理特征及预后的关系。方法:检测156例宫颈癌患者首次治疗前血清CRP水平,以CRP浓度5mg/L为分界点分为CRP正常组和CRP升高组,分析两组间临床病理特征及预后的差异。结果:CRP水平升高组较CRP正常组FIGO分期更晚、淋巴结转移阳性率更高,差异均有统计学意义(P<0.05)。不同血清CRP水平与患者年龄、组织分级、病理类型差异均无统计学意义(P>0.05)。单因素及多因素生存分析均显示FIGO分期晚、淋巴结转移阳性及术前CRP升高是预后危险因素,而年龄、组织分级和病理类型均不是预后的独立危险因素。结论:CRP检测方便、灵敏度高,可作为宫颈癌患者辅助诊断、治疗及预后评价的指标。Objective： To explore the relationship between serum C-reactive protein （CRP） level, clinicopathological characteristics and prognosis of cervical cancer. Methods： Serum CRP levels of 156 patients with cervical cancer were detected prior to therapy, 5 mg/L CRP was designed as demarcation point to divide the patients into normal CRP group and high CRP group; the clinicopatho- logical characteristics and prognosis in the two groups were analyzed and compared. Results： FIGO stage in high CRP group was later than that in normal CRP group, the positive rate of lymph node metastasis in high CRP group was higher than that in normal CRP group, there were statistically significant differences （ P〈0.05 ） . There was no statistically significant difference in serum CRP level among the patients in different age groups, with different histological grades, and different pathological types （P〉0. 05 ） . Univariate and multivariate survival analysis showed that late FIGO stage, positive lymph node metastasis, and increased CRP before operation were risk factors of prognosis of cervical cancer patients, but age, histological grade, and pathological type were not independent risk factors of prognosis of cervical cancer patients. Conclusion： CRP detection is convenient and sensitive, which can be used as assisted diagnosis, treatment, and prognosis evaluation index of cervical cancer patients.
