摘  要:目的:研究胰岛素样生长因子(IGF-Ⅰ、IGF-Ⅱ)、雌激素受体(ER)及孕激素受体(PR)在子宫肌瘤、瘤旁组织及肌层组织的表达,探讨其与子宫肌瘤剔除术后复发的关系。方法:选取笔者医院2012年8~12月确诊为多发性子宫肌瘤并行子宫肌瘤剔除术的66例年轻(手术年龄〈45岁)女性为研究对象,用免疫组织化学方法检测患者子宫肌瘤组织、瘤旁组织及肌层组织中IGF-Ⅰ、IGF-Ⅱ、ER及PR的表达,术后随访1年,按照肌瘤是否复发分为复发组14例,对照组52例,回顾性分析比较两组IGF-Ⅰ、IGF-Ⅱ、ER及PR在3种不同组织中的表达差异。结果:IGF-Ⅰ及IGF-Ⅱ蛋白的阳性表达定位于细胞胞浆中,ER及PR的阳性染色定位于细胞核。肌瘤组织IGF-Ⅰ、IGF-Ⅱ、ER及PR的阳性表达率(59.1%、65.2%、87.9%及75.6%)均显著高于瘤旁组织(21.2%、16.7%、63.6%及68.2%)及肌层组织(12.1%、9.1%、34.8%及28.8%),P〈0.01,瘤旁组织IGF-Ⅰ、IGF-Ⅱ、ER及PR表达均显著高于肌层组织(P值均〈0.05)。两组间比较,复发组的瘤旁组织中IGF-Ⅰ、IGF-Ⅱ及ER表达显著高于对照组(P值均〈0.05),复发组的肌瘤组织中IGF-Ⅱ表达均显著高于对照组(P值均〈0.05),肌层组织中各指标的表达在两组之间无统计学差异(P〉0.05)。结论:IGF-Ⅰ、IGF-Ⅱ、ER及PR可能参与子宫肌瘤的发生。子宫瘤体旁肌层组织IGF-Ⅰ、IGF-Ⅱ、ER及PR的强表达可能与子宫肌瘤的复发有关。Objective: To research the expressions of insulin-like growth factor- I (IGF- I ), insulin-like growth factor- 11 ( IGF- Ⅱ ), estrogen receptor ( ER), and progestrogen receptor (PR) in hysteromyoma, tissue adjacent to hysteromyoma, and myometrium, explore their relationships with relapse of hysteromyoma after myomectomy. Methods: Sixty-six young women ( age at operation was under 45 years old) diagnosed as multiple hysteromyoma definitely and undergoing hysteromyomectomy in the hospital from August to December in 2012 were selected as study objects. Immunohistochemistry was used to detect the expressions of IGF- I , IGF- Ⅱ , ER, and PR in hystero- myoma, tissue adjacent to hysteromyoma, and myometrium, all the patients were followed up for one year after operation, then they were divided into relapse group ( 14 patients ) and control group ( 52 patients ) according to relapse or not, the differences of expressions of IGF- I , IGF- Ⅱ , ER, and PR in hysteromyoma, tissue adjacent to hysteromyoma, and myometrium were analyzed retrospectively. Results: The positive expressions of IGF- I protein and IGF-Ⅱ protein located in cytoplasm. The positive expressions of ER and PR located in cell nucleus. The positive expression rates of IGF- I , IGF-Ⅱ , ER, and PR in hysteromyoma were 59. 1% , 65.2% , 87.9%, and 75.6%, respectively, which were significantly higher than those in tissue adjacent to hysteromyoma (21.2% , 16. 7% , 63.6% , and 68. 2%, respectively) and myometrium ( 12. 1%, 9. 1%, 34. 8%, and 28. 8%, respectively) (P〈0. 01 ) . The positive expression rates of IGF- I , IGF- ]], ER, and PR in tissue adjacent to hysteromyoma were significantly higher than those in myometrium ( P〈0. 05 ) .
