腹腔镜手术治疗胃间质瘤80例临床观察

摘　　要：目的:观察腹腔镜手术治疗胃间质瘤的疗效。方法:选择160例胃间质瘤患者为研究对象,根据手术方式的不同分为观察组和对照组各80例,观察组接受腹腔镜下胃间质瘤切除术,对照组接受开腹胃间质瘤切除术,比较两组的手术情况、术后恢复情况以及炎症反应程度。结果:1手术情况:两组患者的肿瘤定位时间、手术时间无差异,术中出血量观察组明显少于对照组,差异具有统计学意义(P<0.05);2术后恢复情况:观察组的肛门排气时间、引流持续时间、流质饮食时间、卧床时间以及住院总时间均明显短于对照组,差异均有统计学意义(P<0.05);3炎症反应:观察组患者IL-1β、IL-8β、CRP和TNF-α含量均明显低于对照组,差异均有统计学意义(P<0.05)。结论:腹腔镜胃间质瘤切除术有助于减小手术创伤、促进术后恢复、缓解炎症反应,提高治疗效果。Objective To study the efficacy of laparoscopic operation and open operation in treating gastric stromal tumor.Meth-ods 160 cases patients with gastric stromal tumor in our hospital from 2004 April to 2014 April were randomly divided into observation group and control group.Observation group patients received laparoscopic gastric stromal tumor resection,control group patients re-ceived open gastric stromal tumor resection.Surgery operation condition,recovery condition after operation and inflammatory reaction degree were compared.Results （1 ）operation situation：there is no significant difference in tumor localization time and operation time of two groups;observation group bleeding volume during operation were significantly less than that of the control group （36.22 ±5.25 VS 65.55 ±7.18）mL （P〈0.05）;（2）postoperative recovery：postoperative anal exhaust time,drainage duration,liquid diet time,bed time and total hospitalization time were significantly shorter than those of control group （1.38 ±0.19 VS1.91 ±0.25,2.25 ±0.34 VS 3.41 ±0.40,3.19 ±0.55 VS 5.48 ±0.67,3.37 ±0.48 VS 5.71 ±0.70,7.94 ±0.86 VS 11.85 ±1.45）d （P〈0.05）;（3）inflammatory response：IL-1β,IL-8β,CRP and TNF-a content were significantly lower than those of control group （2.17 ±0.22 VS 3.68 ±0.42,86.12 ±9.84 VS 110.45 ±10.21,1.78 ±0.78 VS 3.24 ±1.54,13.21 ±2.64 VS 30.14 ±4.51） ug/L （P〈0.05 ）.Conclusions laparoscopic gastric stromal tumor resection is helpful to reduce operation trauma,promote postopera-tive recovery,reduce inflammation reaction;it’ s an ideal operation mode in treating gastric stromal tumors.
