老年人认知损害相关危险因素及其与C反应蛋白交互作用研究

摘　　要：目的调查老年人认知损害相关危险因素及其与C反应蛋白(C-Reactive Protein,CRP)水平的交互作用。方法对220例老年认知损害患者(研究组)和110例健康对照(对照组)应用中文版的简易智能状态检查量表(C-MMSE)进行评定,并测定其各项代谢指标及血清CRP水平。结果研究组血清CRP高于对照组(P<0.01),C-MMSE评分低于对照组(P<0.01);腹型肥胖、高甘油三酯血症以及高血糖的比例研究组高于对照组(P<0.01)。多因素回归分析提示腹型肥胖、高甘油三酯血症、高血糖以及高CRP是老年认知损害的危险因素(P<0.01)。CRP与高甘油三酯血症之间以及CRP与高血糖之间存在交互作用(P<0.01),其归因比分别为85%和39%。结论老年人认知损害与CRP显著相关,高CRP水平与高甘油三酯血症及高血糖症之间存在交互作用。Objective To investigate the risk factors of cognitive impairment in elderly and the interaction with serum level of C-reactive protein （CRP）.Methods 220 elderly with cognitive impairment （study group）and 110 well matched healthy controls （control group）were assessed with Mini Mental State Examination-Chinese version （C-MMSE）.The metabolic indexes and serum level of CRP were measured in both groups.Results Serum level of CRP in study group was significantly higher than that in control group （P 〈0.01）,and score of C-MMSE in study group was significantly lower than that in control group （P 〈0.01）.Ratios of abdominal obesity,hypertriglyceridemia and hyperglycosemia in study group were significantly higher than those in control group （P 〈0.01 ）.Multiple factor regression analysis indicated that abdominal obesity,hypertriglyceridemia, hyperglycosemia and high serum level of CRP were risk factors of cognitive impairment in elderly （P 〈0.01 ）.Significant interactions were detected between serum level of CRP and hypertriglyceridemia,as well as between serum level of CRP and hyperglycosemia （P 〈0.01;attribution ratio was 85% and 39% respectively）.Conclusion Increased serum level of CRP is associated with cognitive impairment in elderly and is interacted with hypertriglyceridemia,as well as with hyperglycemia.
