水突穴体表电刺激联合穴位注射治疗月经性偏头痛临床效果随机对照观察

摘　　要：目的观察水突穴体表电刺激联合甲氧氯普胺穴位注射治疗月经性偏头痛的临床效果。方法将确诊的月经性偏头痛患者68例随机分为2组。观察组34例给予水突穴体表电刺激联合甲氧氯普胺穴位注射治疗,穴位注射于月经前8 d开始,选取脑户、率谷、百会、头维4个头部穴位分别注射稀释后的甲氧氯普胺1 m L(2.5 mg),隔日注射1次,4次为1个疗程,共3个疗程;水突穴体表电刺激于月经前7 d开始,与穴位注射交替进行,隔日1次,4次为1个疗程,共3个疗程。对照组34例口服氟桂利嗪2.5 mg,每晚1次,疗程3个月。2组均于治疗后3,6个月分别观察镇痛效果、头痛发作频率及时间。结果治疗后2组VAS评分均低于治疗前;发作频率及头痛时间均较治疗前降低或减少,差异均有统计学意义(P<0.01或P<0.05),治疗后观察组VAS评分、头痛发作频率及时间改善情况均优于对照组(P均<0.05)。结论水突穴体表电刺激联合甲氧氯普胺穴位注射治疗月经性偏头痛临床效果确切,且无药物治疗的不良反应,值得研究应用。Objective It is to observe the clinical effect of Shuitu acupoint transcutaneous electricstimulation combined with metoclopramide by acupoint injection on menstrual migraine. Methods 68 patients who were clinically diagnosed menstrual migraine were randomly divided into 2 groups. The observation group with 34 cases was treated with Shuitu acupoint transcutaneous electric stimulation combined with metoclopramide by acupoint injection therapy. Acupoint injection lasted for 8 days before menstruation beginning, lmL metoclopramide diluted （2.5 mg） was injected at the head acupoints including Naohu, Shuaigu, Baihui and Touwei once per two days. 4 times was one course of treatment, and they were totally treated for 3 courses. Shuitu acupoint transcutaneous electric nerve stimulation lasted for 7 days before menstruation beginning. Alternating with acupoint injection. One time every other day, 4 times for a course of treatment, a total of 3 courses. The control group was treated with sibelium 2.5 mg by oral use, once every night. The treatment course were 3 months. In 3, 6 months after treatment, the analgesic effect, headache attack frequency and time were observed in both groups. Results The VAS scores were lower while headache attack frequency and time were less after treatment compared with those before treatment in both groups, the differences were sginficant （P 〈 0.05 or P 〈 0.01 ） , and the improvements of VAS scores, headache attack frequency and time were better in observation group than those in the control group （ all P 〈 0.05 ）. Conclusion Shuitu acupoint transcutaneous electric nerve stimulation combined with metoclopramide by acupoint injection had a good clinical effect on menstrual migraine with no adverse reactions to drug treatment, so they are worthy study and application in clinic.
