52例脑梗死继发癫痫的临床研究

摘　　要：目的研究脑梗死继发癫痫的发作概率、发作时间、发作类型与病灶部位间的关系,探讨其发病机制和治疗方法。方法选取该院2012年10月一2014年6月期间收治的52例脑梗死继发癫痫患者的临床资料,其中男性37例,女性15例,进行回顾性分析,对其发作概率、时间和类型与脑梗死部位之间的关系进行比较。结果在52例患者中,有39例(75%)病变部位位于皮层,还有13例(25%)病变部位在皮层下。脑梗死继发癫痫早发性患者有23例(44.23%),皮层和皮层下比例分别为18:5;脑梗死继发癫痫迟发症状患者为29例(55.77%),皮层和皮层下比例分别为21:8。脑梗死继发癫痫全面性发作的有31例(59.61%),由皮层引起的有23例;部分性发作有21例(43.39%),其中有16例的病灶部位在皮层。结论脑梗死继发癫痫主要是由于皮层病变引起的,其发作时间大都为迟发性,而发作类型多为全面性发作。Objective To study the probability of cerebral seizures of epilepsy, onset time, the relationship between seizure type and lesion site and explore its pathogenesis and treatment.Methods The clinical data of patients with cerebral infarction 52 cases of epilepsy in our hospital from October 2012 to June 2014 were treated, including 37 males and 15 females, were retrospectively analyzed, the probability of its onset, time and type cerebral infarction site and the relationship between the compare.Results In 52 patients, 39 patients （75%） lesions located in the cortex, as well as 13 cases （25%） lesions in the cortex. Infarction patients early onset epilepsy have 23 cases （44.23%）, subcortical and cortical ratio was 18： 5; infarction epilepsy symptoms delayed 29 cases （55.77%）, the proportion of subcortical and cortical were 21： 8. Infarction secondary generalized seizures of epilepsy, 31 cases （59.61%）, caused by the cortex there are 23 cases; partial seizures in 21 cases （43.39%）, of which 16 cases the lesion site in the cortex.Conclusion Epilepsy cerebral cortex is mainly due to most of the lesions caused by the onset time was delayed, and the onset of many types of generalized seizures.
