摘 要:目的:探讨六味地黄汤治疗Ig A肾病的作用机制。方法:实验动物分为正常组、模型组、雷公藤组和六味地黄汤组。除正常组外,均采用牛血清白蛋白(BSA)+脂多糖(LPS)+四氯化碳(CCl4)方法建立实验性Ig AN模型。造模成功后连续予以相应药物灌胃4周,正常组予等体积0.9%Na Cl溶液灌胃。测定各组大鼠24 h尿蛋白;检测血肌酐及尿素氮的含量;用HE和Masson染色法观察肾脏组织形态学改变;免疫组织化学方法检测大鼠肾组织中IL-6和TNF-α的表达。结果:与正常组比较,模型组的24 h尿蛋白明显升高、IL-6和TNF-α表达明显升高(P<0.05);与模型组比较,雷根藤组和六味地黄汤组24 h尿蛋白明显降低,IL-6和TNF-α表达明显降低,差异均有统计学意义(P<0.05);各组大鼠肾功能结果差异无统计学意义(P>0.05);雷公藤组及六味地黄汤组肾小球系膜细胞增生和系膜基质增多情况较模型组明显减轻。结论:六味地黄汤抑制IL-6和TNF-α的表达可能是其治疗Ig A肾病的作用机制之一。Objective: To study the mechanism of Liuwei Dihuang Decoction on IgA Nephropathy. Methods: Experimental animals were divided into normal, model, tripterygium wilfordii and Liuwei Dihuang decoction groups. IgAN rat model was established with bovine serum albumin ( BSA), lipopolysaccharide (LPS) and carbon tetraehloride ( CCl4 ) in all groups except the normal group. After modeling, rats were taken corresponding drugs by intragastric administration for 4 weeks while rats in normal group were taken 0.9% NaCl solution by intragastric administration with the same amount. To determine urine protein, blood creatinine and urea nitrogen in 24h in each group aeeordingly. To observe the pathological changes of renal tissue under light microscope by HE and Masson staining, and to test the expression of IL-6 and TNF-α in the kidney tissues by immunohistochemistry. Results: Compared with the normal group, 24h urine protein, the expression of IL-6and TNF-α increased significantly in the model group( P 〈 0. 055 ). Compared with the model group, 24h urine protein, the expression of IL-6 and TNF-α decreased significantly in Liuwei Dihuang decoction group and tripterygium glycosides group( P 〈 0. 055 ). Each group had no statistical significance of renal function ( P 〉 0. 05 ). Compared with the model group, there is visual reduction in glomerular rnesangial cells proliferation and mesangial metrix increasing in Liuwei Dihuang decoction group and tripterygium glycosides group. Conclusion: Liuwei Dihuang decoction can reduce expression of IL-6 and TNF-α, and it might be one of the effective mechanism for the treatment of IgA Nephrepathy.
