肾色素性副神经节瘤的病例报道并文献复习

摘　　要：目的:探讨肾上腺外色素性副神经节瘤的临床诊断及病理特点。方法:回顾性总结我院收治的1例罕见肾来源色素性副神经节瘤的临床及病理资料,复习文献,对肾上腺外色素性副神经节瘤的临床及病理学特点进行讨论。结果:该患者CT显示左肾中极肾皮质与髓质交界处可见一圆形孤立肿物,包膜完整,增强三期检查病灶均见不均匀强化。行根治性肾切除术,术后病理免疫组化:CD56(+),Cg A(+),HMB45(-),根据病理学检测结果诊断肾来源色素性副神经节瘤。术后随访3年,未见复发转移。复习文献,肾上腺外色素性副神经节瘤十分罕见,临床及影像学检查无特异性,因缺乏大样本随访研究,预后不详。结论:详细的组织病理学分析是诊断及鉴别肾上腺外色素性副神经节瘤的主要方法。Objective ：To summarize and analyze the clinical features and histo-pathological examination of ex- tra-adrenal pigmented paraganglioma.Methods：On the basis of one case report of renal pigmented paragan- glioma,literature regarding extra-adrenal pigmented paraganglioma was reviewed.Results：CT scan revealed a solitary mass located in the central region of the left kidney.Contrast enhanced CT scan revealed iodine con- centration of tumor.Radical nephrectomy was performed.The tumor cells were positive for CD56 and chromo- granin A,negative for HMB-45.Combined with other pathological features, renal pigmented paraganglioma was confirmed.No recurrence or metastasis was found during the 3 years follow-up.In literatures,extra-adrenal pig- mented paraganglioma is a rare tumor.There is no specific of clinical and imaging examination.Comprehensive histopathologieal examination is helpful for diagnosis.Prognosis is unclear because lack of evidence-based re- search.Conclusion：The diagnosis of extra-adrenal pigmented paraganglioma rely on the histopathological exami- nation.
