2型糖尿病合并非酒精性脂肪性肝病相关影响因素分析

摘　　要：目的：探讨2型糖尿病（T2DM）合并非酒精性脂肪性肝病（NAFLD）的主要相关影响因素。方法选取2012年1月~2014年6月北京市大兴区红星医院内科住院的T2DM患者362例，其中单纯T2DM（T2DM组）165例，T2DM合并NAFLD（T2DM＋NAFLD组）197例。收集两组患者的临床资料，检测血糖、糖化血红蛋白、血脂、肝功能、尿酸水平；采用Logistic回归分析筛选T2DM合并NAFLD的主要危险因素。结果与T2DM组比较，T2DM＋NAFLD组的体重指数、腰围、臀围、腰臀比、血压、血糖、糖化血红蛋白、三酰甘油、谷丙转氨酶、谷草转氨酶、γ-谷氨酰转移酶及尿酸水平均升高，高密度脂蛋白胆固醇水平降低，差异有统计学意义（均P〈0.05）。Logistic回归分析结果显示，体重指数、三酰甘油、血糖、糖化血红蛋白和血尿酸是T2DM合并NAFLD的独立危险因素（OR值分别为1.38、1.54、1.14、1.46和1.16，均P〈0.05）。结论肥胖、高尿酸血症、高血糖水平与T2DM合并NAFLD的发病密切相关。Objective To discuss the main related factors of type 2 diabetes mellitus （T2DM） combined with nonalcoholic fatty liver （NAFLD）. Methods 362 cases of T2DM patients hospitalized in Internal Medicine Department of Beijing Daxing District Hongxing Hospital from January 2012 to June 2014 were selected. 165 cases had T2DM （T2DM group）, while 197 cases had T2DM combined with NAFLD （T2DM＋NAFLD group）. Clinical data of patients were collected. Blood sugar, glycosylated hemoglobin, blood fat, liver function, uric acid levels were detected. Logistic regression was used to analyze the major risk factors of T2DM combined with NAFLD. Results Compared with the T2DM group, the body mass index （BMI）, waist circumference, hip circumference, waist-to-hip ratio, blood pressure, blood sugar, glycosylated hemoglobin, triacylglycerol, ALT, AST, GGT and uric acid levels all increased, HDL-C level decreased in the T2DM＋NAFLD group, the differences were statistically significant （P〈0.05）. Results of Logistic regression analysis showed that BMI, triacylglycerol, blood sugar, glycosylated hemoglobin and uric acid were the independent risk factors of T2DM combined with NAFLD （OR=1.38, 1.54, 1.14, 1.46, 1.16, all P〈 0.05）. Conclusion Obesity, high uric acid hematic disease, high blood sugar levels are closely related with T2DM combined with NAFLD.
