摘 要:目的:探讨3.0T MR 扩散张量成像(DTI)及白质纤维束成像(DTT)技术对早期原发性开角型青光眼(POAG)视放射损伤的检测价值。方法收集早期 POAG 患者12例,10例健康志愿者,行3.0T MR-DTI 扫描,比较早期 POAG 患侧眼及正常组视放射纤维束平均长度、纤维束密度、MD 值、FA 值,以 P〈0.05作为具有显著性差异的标准。结果 DTT 后处理软件清晰显示三维视放射形态,POAG 组视放射 FA 值、MD 值、平均纤维束密度、平均纤维束长度、纤维束数目分别为0.45±0.03、(0.85±0.03)×10^-3 mm^2/s、7.59±0.57、(858.10±46.32)mm、195.53±14.90;正常组 FA 值、MD 值、平均纤维束密度、平均纤维束长度、纤维束数目分别为0.53±0.06、(0.83±0.03)×10^-3 mm^2/s、8.04±0.64、(880.33±14.13)mm、201.05±15.28。POAG 组与正常组比较 FA 值明显降低(P〈0.01),MD 值轻度升高(P〈0.05),纤维束密度轻度降低(P <0.05),纤维束长度、纤维束数目差异无统计学意义(P >0.05)。结论 DTI 及 DTT 技术对早期 POAG 视放射的神经损害性具有可靠的诊断价值。Objective To explore the clinical value of MR-DTI and DTT in early evaluation of the lesions on optic radiations in pa-tients with primary open angle glaucoma (POAG ).Methods 12 patients with early stage of POAG and 10 age-matched healthy vol-unteers were examined with DTI sequence on 3.0 T MRI.The fractional anisotropy (FA),mean diffusion coefficient (MD),the av-erage length of fibers,mean fibers/voxel,the number of fibers were compared between the POAG and normal group.The statistic significance is set at P〈0.05.Results The three-dimensional optic radiation was rebuilt by the DTK software package.The MD values, FAvalues,the average length of fibers,mean fibers/voxel,the number of fibers in POAG group were 0.45±0.03,(0.85±0.03)×10-3 mm2/s, 7.59±0.57,(858.10±46.32)mm,1 95.53±14.90,respectively;and those for the normal group were 0.53±0.06,(0.83±0.03)× 10 -3 mm2/s,8.04±0.64,(880.33±14.13)mm,201.05±1 5.28,respectively.The MD values in POAG group were higher,and FA values were lower than normal group,FA value had statistically significant difference between groups (P〈0.01).Mean fibers/voxel in POAG optic radiations were lower than that in normal group.There were some differences in the two groups,but they did not achieve statistical significance in average fiber length and the number of fibers.Conclusion The study confirms that MRI-DTI and DTT can detect the potential neuropathology injuries in the POAG at early stage.
