摘　　要：目的:系统评价国内有关胃癌组织中OPN表达及临床意义的研究。方法:检索CBM、CNKI、VIP和万方数据库,并辅以文献追溯的方法,收集公开发表的所有关于胃癌组织中OPN表达及其与临床病理特征关系的病例对照研究。检索年限均为从建库至2014年8月20日。按纳入排除标准筛选文献并评价纳入研究质量后,应用Rev Man 5.2软件进行Meta分析。结果:共纳入9个病例对照研究,其中胃癌组585例,对照组153例。Meta分析结果显示:OPN在胃癌组与对照组[OR=0.04,95%CI(0.02,0.07)]、高中分化组与低分化组[OR=0.55,95%CI(0.34,0.89)]、临床Ⅰ-Ⅱ期组与临床Ⅲ-Ⅳ期组[OR=0.20,95%CI(0.10,0.38)]、有淋巴结转移组与无淋巴结转移组[OR=3.24,95%CI(1.26,8.33)]的表达差异均有统计学意义。结论:目前国内证据证明,OPN可能参与了胃癌发生、发展的全过程,检测OPN表达有助于判断胃癌患者的预后。Objective：To investigate the expression and clinical significance of osteopontin（ OPN ） in gastric cancer with Meta-analyses. Methods：The published studies were searched in the CBM,CNKI,VIP and WanFang databases,and other relevant journals were also handsearched to identify all the relevant case -control trials. The quality of the included studies was assessed. The Cochrane Collaborationˊs software RevMan 5. 2 was used to test the heterogeneity,overall effect and publication bias of the combined studies. Results：A total of 9 studies were recruited. The positive of OPN in gastric cancer was significantly higher than this in control group tissues with OR（95%CI）at 0. 04（0. 02,0. 07）. The level of OPN expression had a positive correlation with the histological grading,clinical stages with OR（95%CI）,lymph node metastasis,at 0. 55（0. 34,0. 89）,0. 20（0. 10,0. 38）,3. 24（1. 26,8. 33）,respective-ly. Conclusion：ccording to the domestic evidence,OPN may be participate the whole course of carcinogenesis of gas-tric cancer,and may be useful predicting the prognosis of gastric cancer.
